One-third of the population in intensive care units is in a state of circulatory shock, whose rapid recognition and mechanism differentiation are of great importance. The clinical context and physical examination are of great value, but in complex situations as in cardiac care units, it is mandatory the use of advanced hemodynamic monitorization devices, both to determine the main mechanism of shock, as to decide management and guide response to treatment, these devices include pulmonary flotation catheter as the gold standard, as well as more recent techniques including echocardiography and pulmonary ultrasound, among others. This article emphasizes the different shock mechanisms observed in the cardiac care units, with a proposal for approach and treatment.

Objective: To assess the impact of parathyroid gland autotransplantation on the restoration of parathyroid function in patients who are hypoparathyroid after thyroidectomy.

Background Data: Hypoparathyroidism post-thyroidectomy arises when all parathyroid glands are devascularized or injured. Autotransplantation of compromised parathyroids aims to preserve their function and prevent permanent hypoparathyroidism.

Background: Intensive care units (ICUs) are high-stress environments, particularly for nurses, who face numerous stressors that can negatively impact their well-being. This study aimed to examine stress levels and stressors among ICU nurses, investigate their stress coping strategies, and explore the primary stressors in this demanding work environment.

Materials And Methods: Employing a cross-sectional design, this study assessed the stress levels of ICU nurses by using the Perceived Stress Scale and their coping strategies through the Brief-COPE scale, from March 15, 2021, to April 14, 2021.

Background: This study examined the effects of yoga-based educational interventions on the volume and composition of breast milk in premature infants' mothers admitted to the neonatal intensive care unit (NICU).

Materials And Methods: A randomized controlled trial was conducted on 78 primiparous mothers whose premature infants were less than 34 weeks and were hospitalized in the NICU of Ayatollah Rouhani Hospital from February 2021 to November 2022. Mothers were assigned to a control group and an experimental group, that is, yoga, using the block randomization method.

Pelvic sheet binders: Are doctors placing them in the correct position?

Afr J Emerg Med

December 2024

Division of Emergency Medicine, Faculty of Health Sciences, School of Clinical Medicine, University of the Witwatersrand, Johannesburg, South Africa.

Introduction: Unstable pelvic fractures cause significant bleeding, morbidity, and mortality. Commercially available Pelvic Circumferential Compression Devices (PCCDs) are used in the initial resuscitation and management of these cases. In the trauma-burdened, resource limited setting of Southern Africa, the available alternative is a pelvic sheet binder (PSB).

Background: While prolonged operative time and increased levels fused have been shown to increase the risk of prolonged intensive care unit (ICU) length-of-stay (LOS), studies are limited in guiding decision-making regarding the need for intensive care postoperatively. This is especially the case among the cohort of adolescent idiopathic scoliosis (AIS) patients undergoing posterior spinal fusion (PSF); associations between comorbidities and ICU LOS are not well-delineated.

Methods: AIS patients who underwent PSF from January 1st, 2016 to December 1st, 2016 at 101 participating centers were identified using the American College of Surgeons (ACS) National Surgical Quality Im-provement Project (NSQIP) Pediatric database.
