Fibrosing mediastinitis is a rare disorder characterized by the invasive proliferation of fibrous tissue within the mediastinum. This fibrosis can result in compression of intrathoracic structures including the pulmonary vasculature leading to clinical symptoms and complications like pulmonary hypertension. Here, we present two cases of young patients with fibrosing mediastinitis complicated by pulmonary artery stenosis requiring medical and endovascular management with excellent results.

We present the case of a 33-year-old patient with atypical pulmonary anatomy secondary to fibrosing mediastinitis who underwent pulmonary thrombectomy with the Lightning Flash aspiration catheter following a pulmonary embolism triggered by tibial osteomyelitis. Despite an inconclusive computed tomography angiography report, the diagnosis was made through clinical evaluation associated with electrocardiographic and echocardiographic changes. Angiography was crucial for diagnosis.

IgG4-releated disease is systemic fibroinflammatory disease characterized by infiltration of IgG4-positive plasma cells in the affected tissue. Mediastinal fibrosis, which is also known as fibrosing mediastinitis, is a rare form of IgG4-releated disease. We present a case of mediastinal and retroperitoneal fibrosis in 52-year-old man who was detected with increased radiopharmaceutical uptake in 18F-FDG PET/CT imaging.
