Transvenous arteriovenous malformation embolization.

Transvenous embolization of arteriovenous malformations has emerged as a safe and effective intervention. In carefully selected patients the transvenous approach has shown efficacy similar to that of conventional transarterial routes. The optimal lesions amenable to the transvenous approach have not been robustly delineated; however, the approach is reserved for small, deep seeded, single venous drainage lesions with poorly amenable arterial vascular supply. In this video we demonstrate a case of transvernous Onyx embolization of a Spetzler-Martin grade 3 (SM3) left hemispheric, ruptured arteriovenous malformation. The patient underwent successful, single session, complete embolization of the arteriovenous malformation through a venous route without further complications or repeat treatment.

Unlabelled: Brain arteriovenous malformations (AVMs) pose a significant treatment challenge, with current options including microsurgical resection, endovascular embolization, radiosurgery, or combinations thereof. Here, we present our experience with a curative strategy combining complete endovascular treatment followed by microsurgical resection under the same anesthesia session, without relying on a hybrid operating room.

Methods: We reviewed consecutive AVM patients who underwent endovascular treatment (EVT) and microsurgical resection (MS) in a single anesthesia session (COMBI-AVM protocol) from December 2017 to July 2022.

Peripheral Arteriovenous Malformations (pAVMs) are congenital vascular anomalies characterised by abnormal connections between arteries and veins that bypass the capillary network. This bypass results on a high-flow and low resistance vascular structure termed nidus. The high-flow and complex angioarchitecture of pAVMs makes treatment challenging and often suboptimal, as evidenced by high recurrence rates.

Background: Arteriovenous malformations (AVMs) present a significant therapeutic challenge, as current treatment modalities frequently fail to achieve complete and rapid obliteration and are associated with substantial morbidity in both the short and long term. This underscores the critical need for innovative therapeutic strategies that enable efficient AVM obliteration while minimizing patient risk. The current review aims to comprehensively assess the role of ATME in AVM management, examining its clinical efficacy, associated risks and benefits, and the economic and ethical implications to provide valuable foundation for future studies and guiding development in treatment strategies for AVMs.

Laser speckle flowgraphy (LSFG) offers a non-invasive, accessible means to evaluate ocular blood flow. A case of a woman in her 70s presenting with 6 months of ocular injection following cataract surgery is presented in this report. LSFG measurements indicated lower mean blur rate (MBR) values in the affected eye compared with the healthy eye, though a decrease was also noted in the healthy eye.
