The article presents the results of a comparative analysis of the level of interleukins of children with Shigellosis in different periods of the disease depending on the presents of CMV-infection in patients. To achieve the goal, 63 children aged from three to eight years old with Shigellosis were examined, who were treated at the Regional Children's Infectious Clinical Hospital of Kharkiv, Ukraine. It has been established that the presence of CMV in patients with Shigellosis leads to the development of cytokine imbalance which persists until the period of early convalescence. In the acute period of Shigellosis in children with CMV infection, the level of IL-1β significantly increases with unreliable fluctuations in the indices of TNF-α and IL-4. In children with Shigellosis without comorbidities, an increase in the level of all cytokines is recorded. In children with monoinfection to the period of convalescence, a significant dynamics of decrease in the levels of all interleukins was observed with their indicators approaching the physiological norm. In case of Shigellosis in children with CMV, the content of IL-1β and IL-4 was still significantly high. Violation of the normalization of the cytokine profile of patients with Shigellosis on the background of CMV infection may be one of the leading factors in the torpid clinical recovery, the formation of an unfavorable course of the disease. In our opinion, these features of the cytokine response of patients must be considered during the period of rehabilitation and follow-up.

Background: Salmonella and Shigella are major enteric pathogens that cause diarrhea in children worldwide. They are pathogenic microbes that cause significant diarrheal morbidity and mortality in under five children in resource limited countries. Thus, this systemic review and meta-analysis aimed to investigate the prevalence of Salmonella and Shigella species and their multidrug resistance patterns in pediatric populations in Ethiopia.

With increasing antibiotic resistance in gram-negative bacteria, including those causing Shigellosis, evidence of safety and pharmacokinetics data on new oral antibiotics is crucial. We aimed to investigate the safety and pharmacokinetic properties of an oral carbapenem, tebipenem pivoxil, along with it's ability to produce desired results in childhood shigellosis. This randomized pilot clinical trial was conducted at Dhaka Hospital, icddr,b in 2022 between May and September.

A Recombinant Strain Expressing ETEC Heat-Labile Enterotoxin B Subunit Shows Promise for Vaccine Development via OMVs.

Diarrheal diseases caused by and enterotoxigenic (ETEC) are significant health burdens, especially in resource-limited regions with high child mortality. In response to the lack of licensed vaccines and rising antibiotic resistance for these pathogens, this study developed a recombinant strain with the novel incorporation of the gene for the heat-labile enterotoxin B (LTB) subunit of ETEC directly into 's genome, enhancing stability and consistent production. This approach combines the immunogenic potential of LTB with the antigen delivery properties of outer membrane vesicles (OMVs), aiming to provide cross-protection against both bacterial pathogens in a stable, non-replicating vaccine platform.

Article Synopsis
  • Serotype 6 is a common cause of moderate to severe diarrhea but remains understudied, prompting research into its genomic and phenotypic characteristics compared to other serotypes.
  • Genomic analyses revealed notable similarities among 6 strains across different regions and timeframes, along with the identification of a potential novel virulence factor and unique patterns of antibiotic susceptibility specific to geographic locations.
  • Findings suggest that serotype 6 has distinct genetic and phenotypic traits that could enhance vaccine development and diagnostic tools, which is crucial given the rising incidence of shigellosis, particularly in low- and middle-income countries.
