OCT in a 74-year-old woman highlights how an angiographic coronary fenestration can be acquired through calcific nodule atherosclerosis in the coronary arteries. We demonstrate how tandem anatomic and physiologic assessment can be used to diagnose, functionally evaluate, and effectively treat this rare finding.

Objective: Open surgical suprarenal aortic fenestration (OSSAF) is a technique to treat complicated type B aortic dissection (cTBAD) by resecting the intimal membrane at the level of the visceral arteries. This invasive procedure is largely abandoned since the advent of thoracic endovascular aortic repair (TEVAR) as becoming the gold standard of treating cTBAD. Identifying patterns in the late history of patients who underwent OSSAF might help better understand the evolution of TBAD.

Importance: Fenestrated and branched endovascular aortic repairs (F/BEVAR) have been adopted by many centers. However, national trends of F/BEVAR use remain unclear, particularly at sites who perform them without an US Food and Drug Adminstration (FDA)-approved investigational device exemption (IDE).
