This case report highlights the incorporation of a tenon-mortise type of nonrigid connector between two adjacent pontics in a fixed functional space maintainer, to allow limited passive eruption and alignment of young permanent teeth. A 13-year-old male patient had lost his lower second premolar and first molar on the right side, secondary to the surgical removal of an odontogenic keratocyst a year ago. A space maintainer had to be placed until implants, or a fixed partial denture could be placed, after the completion of his growth. The appliance consisted of band type retainers with rests on the first premolar and the second molar, rigidly connected to their adjacent pontics (second premolar and first molar), with a tenon-mortise type of connector between the pontics. The casting was composed of nickel-chromium, out of which the pontics were layered with ceramic.

The mixed dentition stage is a vital period characterized by significant physiological changes, including jaw growth, the development and eruption of permanent teeth, the exfoliation of primary teeth, and the maturation of surrounding soft tissues. These processes collectively ensure functional, esthetic, and stable occlusion. Disruptions during this stage, such as the premature loss of deciduous teeth, can lead to spacing or crowding issues and affect the dental arch length and the position of permanent teeth.

Unlabelled: Preserving deciduous teeth till their natural exfoliation is important to maintain arch length in different dentition periods. Premature loss of primary teeth may cause discrepancy in arch length due to drifting of teeth and supraeruption of the opposite teeth, thereby hindering the development of a harmonious, functionally and esthetically acceptable dentition. This article describes the fabrication and clinical application of a modified functional distal shoe appliance used due to pathological obliteration of the primary/deciduous mandibular second molar prior to the eruption of the permanent mandibular first molar.
