AI Article Synopsis

  • A 57-year-old male on HIV PrEP experienced sudden wrist pain and swelling, which was linked to a disseminated gonococcal infection (DGI) causing purulent arthritis.
  • The case highlights how DGI, while rare (under 3% of infections), can present with unusual symptoms and emphasizes the need for healthcare providers to be aware of these extragenital manifestations in patients on PrEP.
  • There has been an increase in gonococcal infections recently, particularly among individuals taking PrEP, making awareness crucial for diagnosing septic bacterial arthritis related to DGI.

Article Abstract

We present a case of a generally healthy, immunocompetent, 57 year old male on HIV pre-exposure prophylaxis (PrEP) who developed the sudden onset of right wrist pain and swelling with associated left second finger tenosynovitis, due to a disseminated gonococcal infection (DGI) with purulent arthritis. This case's aim is to demonstrate a well-documented but infrequently seen manifestation of infection and to highlight the need for awareness of extragenital manifestations of gonococcal infection in the era of HIV PrEP. DGI accounts for less than 3 % of infections, with purulent arthritis being one presentation. The incidence of infections has increased in recent years and is high among those taking PrEP. Providers should be aware of the possibility of DGI and as a potential etiologic agent in septic bacterial arthritis.

Objectives: It has been suggested that the emergence of ceftriaxone-resistant strains of Neisseria gonorrhoeae involves the incorporation of the penA gene from commensal Neisseria spp. that are resistant to ceftriaxone. However, the mechanism of this mosaic penA generation is unknown.

Background: Effective strategies to reduce sexually transmitted infection burden and transmission among female sex workers (FSWs) and their networks are needed. We report sexually transmitted infection prevalence among FSWs in Zimbabwe and investigate the performance of screening algorithms.

Methods: Respondent-driven sampling (RDS) surveys, including blood sampling for syphilis serology, were conducted among FSWs in 3 communities in Zimbabwe in 2017.

The progressive development of resistance in to almost all available antibiotics has made it crucial to develop novel approaches to tackling multi-drug resistance (MDR). One of the primary causes of antibiotic resistance is the over-expression of the MtrCDE efflux pump protein, making this protein a vital target for fighting against antimicrobial resistance (AMR) in . This study was aimed at evaluating the potential MtrCDE efflux pump inhibitors (EPIs) and their stability in treating gonorrhoea infection.
