We present a case of antidromic AVRT involving an atriofascicular pathway in a patient with an atretic coronary sinus os. This patient had left ventricular dysfunction with rate-related left bundle branch block aberrancy in sinus rhythm. In the electrophysiology lab, there was VA dissociation for a few beats during antidromic tachycardia.

Painful left bundle branch block (LBBB) syndrome is a rare cause of episodic chest pain associated with transient LBBB in the absence of flow-limiting coronary artery disease and myocardial ischaemia on functional testing. The aetiology of this phenomenon is not clear, but in many reported cases, these transient episodes of LBBB are rate related. The mechanism of chest pain is not well understood.

Background: Acute limb ischaemia usually is caused by a blood clot blocking an artery or a bypass graft. Severe acute ischaemia will lead to irreversible damage to muscles and nerves if blood flow is not restored in a few hours.
