Purpose: Related to the development and diffusion of ALIF and XLIF, it is possible to correct sagittal malalignment in selected cases of lumbar degenerative discopathy with a relatively low invasiveness. Still, the malposition or the inappropriate size of the implanted cages may lead to the subsidence of the vertebral endplates with loss of correction as well as a decrease in the potential to restore spinal biomechanics in the long run. The aim of this study is to evaluate safety, feasibility, and preliminary clinical and radiological results when using custom-made, trabecular titanium cages in ALIF and XLIF procedures.

Methods: We prospectively evaluated 18 consecutive patients who underwent either an ALIF or an XLIF procedure with the implant of a custom-made, trabecular titanium cage for lumbar degenerative disease with sagittal imbalance, with a minimum of 1-year clinical and radiological follow-up.

Results: After a mean follow-up of 14 months, the Oswestry score dropped to a mean of 13 from a preoperative value of 48 (p < 0.0001). Lumbar lordosis was significantly improved, especially in the lower lumbar segment L4-S1 (+ 11 ± 7°; p < 0.0001). No cases of subsidence were noted.

Conclusions: Custom-made, trabecular titanium cages allowed a segmental, steady, durable sagittal correction via ALIF and XLIF approaches. The absence of cage subsidence at 1 year encourages further studies on a larger cohort with longer follow-up. These slides can be retrieved under Electronic Supplementary Material.

Article Synopsis
  • Human mesenchymal stem cells (hMSCs) respond to mechanical and biophysical signals, with pulsed electromagnetic field (PEMF) stimulation showing promise in treating musculoskeletal disorders and enhancing bone tissue engineering.
  • This study examines the effects of PEMF on bone regeneration by creating trabecular bone-like tissues, culturing them in a bioreactor, and analyzing cellular signaling pathways through transcriptomic studies.
  • Results indicate that PEMF boosts hMSC immune responses and stimulates processes like angiogenesis and osteogenesis, reflecting key aspects of bone healing, showcasing the potential of the bioreactor platform for future research.

Article Synopsis
  • Previous methods for culturing bone used polycarbonate bioreactors, which were hard to make and limited in size, prompting the development of a new 3D printed MED610 bioreactor.
  • This study involved culturing trabecular bone cores from bovine sternum in the MED610 bioreactors, comparing groups subjected to mechanical loading versus control.
  • Results showed the Load group's apparent elastic modulus increased significantly more than the control group over 21 days, indicating the new bioreactor effectively supports bone adaptation, but larger studies are needed for more robust conclusions.

[Continuous Optimisation of Experimental Models - an Example from Glaucoma Research].

Klin Monbl Augenheilkd

January 2024

Augenheilkunde, ARTEMIS Augen- und Laserzentrum Frankfurt, Deutschland.

Background: There is a great demand for suitable models to test novel surgical and therapeutic approaches in glaucoma therapy. To address this need and to provide further alternatives to in vivo animal models, we aimed at modifying an established in vitro porcine eye perfusion model.

Methods: Two weaknesses of the previously established porcine anterior segment model include media leakage during perfusion and setup disintegration due to mechanical instability.

Background/aims: Professional and amateur athletes might have veneer restorations. The aim of this study was to investigate the protective effect of mouthguards on veneered anterior restorations.

Methods: A nonlinear dynamic analysis was performed to simulate conditions during an impact with or without a custom-made mouthguard.

Introduction: Glaucoma is a chronic, progressive disease of the optic nerve that can cause vision loss and blindness. High-frequency deep sclerotomy (HFDS) is a novel ab interno procedure used to lower intraocular pressure (IOP). This study aims at examining hypotensive effects of HFDS in patients with medically uncontrolled primary open-angle glaucoma (POAG).
