Diagnosis and Treatment of Oncologic Emergencies.

Radiol Technol

Published: November 2019

The incidence of cancer is increasing; however, mortality rates have decreased in the past 30 years, largely because of increased awareness and early detection methods. The majority of cancer cases are treated as serious conditions, but not all are considered urgent. Some cases, known as oncologic emergencies, are identified as critical. This article discusses the diagnosis and treatment of common and potentially life-threatening oncologic emergencies such as metabolic, hematologic, and structural emergencies, as well as the role of the radiologic technologist and cancer care team.

Introduction: Prostate cancer represents a significant oncological challenge, with its natural history predominantly driven by androgen receptor (AR) signaling. The pivotal role of this pathway underscores the rationale for targeting AR activity in therapeutic strategies. However, the development of resistance mechanisms has highlighted the need for advanced therapies to address the complexity of the castration-resistant status.

Cervical cancer is one of the most common and challenging malignancies diagnosed during pregnancy, requiring a complex balance between effective oncological management and pregnancy preservation. This narrative review synthesizes current evidence and clinical experience regarding the management of cervical cancer in pregnant patients. While treatment should generally follow standard protocols for nonpregnant patients, emerging data suggest that pregnancy can often be safely maintained without compromising oncological outcomes.

Background: Hepatic artery infusion with floxuridine is a treatment option for patients with colorectal liver metastases or intrahepatic cholangiocarcinoma. Outcomes from newer centers are understudied. Predictive markers are needed, and quantitative circulating tumor DNA is an emerging candidate method for predicting response in patients receiving hepatic artery infusion.

Fluorescence imaging technologies have revolutionized vascular surgery by enabling real-time visualization of vascular anatomy, blood circulation, and tissue perfusion, thus improving intraoperative decision-making. This review provides a comprehensive analysis of key fluorescence modalities, including Fluorescence-Guided Surgery (FGS), Near-Infrared (NIR) fluorescence imaging, and Indocyanine Green (ICG) angiography, highlighting their roles in optimizing tissue perfusion assessment, vessel patency evaluation, and identifying anatomical variations. Unlike existing literature, this review addresses critical gaps in current practices by comparing these technologies and exploring their applications across a range of vascular procedures such as peripheral vascular surgery, coronary artery bypass grafting, and oncological operations.

Approximately half of melanoma patients relapse or fail to respond to current standards of care, highlighting the need for new treatment options. Engineering T-cells with chimeric antigen receptors (CARs) has revolutionized the treatment of hematological malignancies but has been clinically less effective in solid tumors. We therefore sought to engineer alternative immune cell types to inhibit melanoma progression.
