The Role of Transnasal Endoscopy in Diet Management of Eosinophilic Esophagitis.

ACG Case Rep J

December 2024

Gastrointestinal Eosinophilic Diseases Program, Department of Pediatrics, University of Colorado School of Medicine, Aurora, CO.

The diagnosis and monitoring of eosinophilic esophagitis (EoE) relies on clinical symptoms and histological examination of the esophageal mucosa. Currently, esophagogastroduodenoscopy with biopsy is the standard of care in the diagnosis and monitoring of EoE. Unsedated transnasal endoscopy (TNE) is a less invasive and less costly procedure that can be performed without general anesthesia.

Article Synopsis
  • Patients with esophageal atresia and tracheoesophageal fistula (EA-TEF) face various gastrointestinal complications, necessitating potential lifetime endoscopic monitoring.
  • There's growing interest in using pediatric unsedated transnasal endoscopy (TNE) as a safer, cost-effective alternative to traditional endoscopy for these patients.
  • The report highlights four cases where TNE was effectively utilized for monitoring and managing conditions like gastroesophageal reflux and eosinophilic esophagitis, supporting its use in routine screenings as per guidelines.

Esophageal submucosal hematoma is a rare, often incidental complication of therapeutic endoscopic procedures marked by disrupted blood vessels beneath the esophageal mucosa, forming a hematoma. We report the unique case of a severely thin and alcoholic 38-year-old woman with a history of reflux esophagitis who developed an esophageal submucosal hematoma during an unsedated transnasal endoscopy for health check-up. During the procedure, the patient experienced strong vomiting reflexes and vomited blood, leading to the initial suspicion of either Mallory-Weiss syndrome or epistaxis.

Pediatric inflammatory bowel disease (IBD) is a chronic inflammatory disorder, with increasing incidence and prevalence worldwide. There have been recent advances in imaging and endoscopic technology for disease diagnosis, treatment, and monitoring. Intestinal ultrasound, including transabdominal, transperineal, and endoscopic, has been emerging for the assessment of transmural bowel inflammation and disease complications ( fistula, abscess).

Introduction: Unsedated transnasal endoscopy (TNE) as transnasal esophagoscopy (TN-Eso) has emerged as a promising alternative to esophagogastroduodenoscopy (EGD) under sedation to assess eosinophilic esophagitis (EoE). We report our center's experience using single-use gastroscopes to perform sedation-free transnasal EGD (TN-EGD) with biopsies in an office-based setting.

Methods: A retrospective review was performed on patients with eosinophilic esophagitis who underwent office-based sedation-free TNE with topical analgesia and virtual reality (VR) procedural dissociation and distraction.
