Can cultural representations be used to therapeutic effect in the treatment of mood disorders like depression and anxiety? This article develops a theoretical framework that outlines how this might be achieved by way of mid-level cultural metrics that allow otherwise heterogeneous forms of representation to be grouped together. Its prediction is that abstract representations-as measured by Shannon entropy-will impact positively on anxiety, where concrete representations will positively impact on depression. The background to the prediction comes from construal level theory, a branch of social psychology that deals with the effects of abstraction on psychological distance; the types of cultural representations analysed include image, narrative and film. With a view to evaluating the hypothesis, the article surveys the empirical literature in art therapy, creative bibliotherapy and cinema therapy.

The current DSM-oriented diagnostic paradigm has introduced the issue of heterogeneity, as it fails to account for the identification of the neurological processes underlying mental illnesses, which affects the precision of treatment. The Research Domain Criteria (RDoC) framework serves as a recognized approach to addressing this heterogeneity, and several assessment and translation techniques have been proposed. Among these methods, transforming RDoC scores from electronic medical records (EMR) using Natural Language Processing (NLP) has emerged as a suitable technique, demonstrating clinical effectiveness.

At least 227 combinations of symptoms meet the criteria for Major Depressive Disorder (MDD). However, in clinical practice, patients consistently present symptoms in a regular rather than random manner, and the neural basis underlying the MDD subtypes remains unclear. Depression is one of the most disabling mental disorders worldwide and characterized by symptoms including worthlessness, anhedonia, sleep, and appetite disturbances. Recently, studies have suggested that tryptophan (Trp) metabolism plays a key role in depressed mood through serotonin and kynurenine pathway involving enzyme tryptophan 5-monooxygenase (TPH) and indoleamine-2,3-dioxygenase (IDO) respectively. Moreover, during neuroinflammation, IDO is activated by proinflammatory cytokines and affects neurogenesis, cognition, disturbed hypothalamic-pituitary-adrenal (HPA) axis, and gut homeostasis by altering the gut bacteria and its metabolites like Trp derivatives.
