Purpose: To evaluate the efficacy and safety of the Triple-P procedure as a conservative method in women with morbidly adherent placenta (MAP).

Materials And Methods: A prospective trial conducted on 20 women performing elective cesarean sections (CS) at 37 weeks for anterior placenta previa accreta or increta. All women were young aged with low parity and previous CS deliveries. Triple-P procedure involved delivery of the fetus through a uterine incision placed above the upper border of the placenta, bilateral uterine arteries ligation immediately after delivery of the fetus followed by placental nonseparation and myometrial excision with reconstruction of the uterine wall in a T-shaped manner. The study outcome measures included duration of surgery, amount of intra and postoperative blood loss, Percentage of hemoglobin (Hb %) reduction, the need to perform hysterectomy and postoperative complications.

Results: Mean duration of surgery was 58 ± 1.8 min, mean intraoperative blood loss was 1.3 ± 0.3 l, mean postoperative blood loss was 180 ± 94 ml and mean Hb % reduction was 1.5 ± 0.1 g/dl. Only one case necessitated hysterectomy for severe bleeding.

Conclusion: Triple-P procedure is a novel effective weapon that can replace hysterectomy in suitable women with MAP, especially in young patients with low parity.

Background: Placenta Accreta Spectrum (PAS) disorders has been reported to be associated with a maternal mortality rate of 7-10%, worldwide, and many women who survive, experience life changing morbidity. Triple P procedure (- perioperative placental localization and incision on the myometrium above the upper border of the placenta; - pelvic devascularisation; and -placental non-separation and myometrial excision) was developed in 2010 as a novel conservative alternative to peripartum hysterectomy to avoid severe maternal morbidity and mortality). There have been several modifications to the original Triple P Procedure to achieve "pelvic devascularisation" based on locally available resources.

Unsupervised Domain Adaptation (UDA) has shown promise in Scene Text Recognition (STR) by facilitating knowledge transfer from labeled synthetic text (source) to more challenging unlabeled real scene text (target). However, existing UDA-based STR methods fully rely on the pseudo-labels of target samples, which ignores the impact of domain gaps (inter-domain noise) and various natural environments (intra-domain noise), resulting in poor pseudo-label quality. In this paper, we propose a novel noisy-aware unsupervised domain adaptation framework tailored for STR, which aims to enhance model robustness against both inter- and intra-domain noise, thereby providing more precise pseudo-labels for target samples.

Background: Parenting interventions are crucial for promoting family well-being, reducing violence against children, and improving child development outcomes; however, scaling these programs remains a challenge. Prior reviews have characterized the feasibility, acceptability, and effectiveness of other more robust forms of digital parenting interventions (eg, via the web, mobile apps, and videoconferencing). Recently, chatbot technology has emerged as a possible mode for adapting and delivering parenting programs to larger populations (eg, Parenting for Lifelong Health, Incredible Years, and Triple P Parenting).

Article Synopsis
  • Germany and the EU have seen over 1.6 million refugees since 2014, with many facing challenges like discrimination, poor living conditions, and mental health issues, especially among parents.
  • The study aims to improve mental health in refugee parents through a randomized controlled trial involving 188 participants, providing a 10-week intervention focused on parenting skills and mental health support, compared to standard treatment.
  • The research is ethically approved and will assess outcomes at multiple stages, running from February 2019 to July 2025, to evaluate both parental and child mental health improvements.

Background: Clinicians and researchers should consider the expected benefits and potential harms of an intervention. Parenting programmes are a widely used evidence-based intervention for child behaviour problems. However, few data are available on potential negative effects.
