Objective: The middle fossa craniotomy for tegmen defect repair provides wide access. This approach often requires temporal lobe manipulation, lumbar drain placement, and longer recovery. We describe a keyhole middle fossa approach with a simple titanium skull base repair that allows for wide access with no temporal lobe manipulation and does not require lumbar drain placement, which results in a dramatic reduction in hospital length of stay.

Methods: A retrospective review was performed on 14 consecutive patients with spontaneous cerebrospinal fluid (CSF) otorrhea. Each patient underwent a keyhole middle fossa approach followed by multilayer dural repair with titanium mesh "gull wing" skull base reconstruction. Postoperative measures included operative time, length of hospital stay, CSF leak recurrence, and surgical complications (seizures, hemorrhage, aphasia, infection).

Results: The average age of the patients was 60.7 ± 12.7 years old, and average body mass index was 32.8 ± 7.9 kg/m. Nine of the patients were female. The average operative time was 103 ± 32.8 minutes. The average hospital length of stay was 1.4 days. There were no cases of postoperative CSF otorrhea, meningitis, aphasia, or seizures. There were no recurrences over a mean follow-up of 20.3 months (range: 5-48 months).

Conclusions: A minimally invasive keyhole middle fossa approach with a multilayer dural reconstruction including titanium mesh "gull wing" skull base repair provides a quick, effective treatment for a broad spectrum of tegmen defects and meningoencephaloceles. This exposure and reconstruction technique do not require the use of a lumbar drain and result in minimal hospitalization.

To investigate the clinicopathological features, diagnosis, genetic alterations, and biological behaviors of hamartomatous inverted hyperplastic polyp (HIHP) in the gastrointestinal tract. The clinical, sonographic, endoscopic and pathologic data of 10 HIHP cases diagnosed at the First Affiliated Hospital of Air Force Medical University, Xi'an, China from January 2013 to March 2024 were collected. Their clinicopathological features and histological morphology were analyzed.

: The aim was to evaluate vestibular function in patients with unilateral vestibular schwannoma before and in the short and medium term after surgical treatment to analyze vestibular compensation. The identification of the prognostic factors determining incomplete and slower balance recovery was assessed. Forty-five patients with unilateral vestibular schwannoma treated surgically through the middle cranial fossa and translabyrinthine approach were enrolled in this study.

A morphometric analysis of the cranial base in trigonocephaly.

Department of Clinical Anatomy, School of Laboratory Medicine and Medical Sciences, University of KwaZulu-Natal, Durban, South Africa. Electronic address:

Trigonocephaly occurs when the metopic suture fuses prematurely. Few studies have documented the morphometry of the entire anterior cranium in trigonocephaly and not on the morphometric changes to the cranial fossae alone. Thus, this study aimed to determine and compare the dimensions of the anterior cranial fossa (ACF) in trigonocephaly and control groups.

Aim: The Transorbital and supraorbital minimally invasive approaches have been defined to reach intraorbital structures, adjacent sinuses, skull base, and other intracranial targets in this region. These approaches reduce the possible cosmetic and brain retraction-related morbidities caused by traditional transcranial approaches. Although these pathways are being studied endoscopically, a stereotactic approach has not been defined.

The purpose of the current study was to determine the angulation of the dural venous sinuses in soft tissue, to evaluate differences between types of tissue, and to discuss the potential influence of these angulations on intracranial venous hemodynamics and related pathologies. Angulations formed in different segments of the transverse, sigmoid, and superior sagittal sinuses were measured in 13 adult human cadaveric heads (26 sides). After the soft tissues were removed, measurements were also taken from the underlying bone.
