Purpose: This report aims to document a long-term follow-up of a rare case of Brown-McLean syndrome where high-density central corneal endothelial cells (CECs) were maintained, yet the peripheral area developed corneal edema.
Observations: A 72-year-old Japanese male was referred to our hospital. He had a history of three instances of rhegmatogenous retinal detachment in the right eye, requiring three intraocular surgeries.

Purpose: Brown-McLean syndrome (BMS) is a clinical condition characterized by peripheral corneal edema with central corneal transparency. This study aims to document the tomographic and biomechanical characteristics of 3 patients with typical BMS features using the Pentacam® AXL and CORVIS ST® (Oculus Optikgeräte GmbH, Wetzlar, Germany).
Observations: Three cases of BMS are presented.

Brown-McLean syndrome not reported in association with homocystinuria and generally occurs in patients with long-term aphakia. Presented as peripheral corneal edema with central clear cornea tends to spare vision without the need of any surgical intervention. The purpose of reporting such a rare condition as a first case of Brown-McLean syndrome associated with systemic disease of homocystinuria.
