Gastric aspiration, which can cause acute, diffuse, inflammatory lung injury, is of particular concern in critically ill patients. This study aimed to determine the effects of immediate prone positioning on the degree of lung injury and inflammatory response induced by gastric aspiration. Following induction of gastric aspiration by injection of gastric fluid, 16 healthy pigs were randomized to one of two groups: supine position (SP) or prone position (PP). After ventilation and monitoring for 6 hr, all pigs were euthanized. The ratio of the partial pressure of arterial oxygen/fraction of inspired oxygen (P/F) and the partial pressure of arterial carbon dioxide (P) were recorded during the 6-hr study period. Serum levels of interleukin (IL)-6 were measured every 2 hr, and the mean optical density (MOD) of IL-6 in lung tissues and lung-injury scores were measured at the end of the experiment. The PP group showed a significantly higher P/F ratio, lower serum IL-6 concentration ( = .015), lower lung-injury scores ( = .012), and lower IL-6 concentration and MOD of IL-6 in lung tissue, especially in dorsal ( = .001, = .021, respectively) and nondependent regions ( = .005, = .035, respectively) than the SP group. There were no statistically significant differences in P between the groups. Lung-injury severity was positively correlated with the IL-6 concentration and MOD of IL-6 in lung tissues ( < .05). These results suggest that immediate prone positioning alleviated the degree of aspiration-induced lung injury, possibly through mitigating IL-6-mediated lung inflammation.

Cavitary Pulmonary Tuberculosis in an Infant.

Cureus

November 2024

Pediatrics, Unidade Local de Saúde São João, Porto, PRT.

Tuberculosis (TB) continues to pose a significant health challenge globally and in Portugal. Diagnostic challenges persist, especially in infants, where TB often presents with atypical symptoms. A previously healthy three-month-old male infant from Vila Nova de Famalicão, Portugal, was admitted with cough, rhinorrhea, respiratory distress, and high-grade fever.

In this editorial, we reviewed the article by Li . We aimed to explore various perspectives to further mitigate the risk factors for postoperative nausea and vomiting (PONV), which could significantly reduce its incidence and related postoperative complications. PONV is highly prevalent among patients undergoing bariatric surgery, yet there are relatively few related studies.

The role of MAPK pathway in gastric cancer: unveiling molecular crosstalk and therapeutic prospects.

Gastric cancer remains a significant health burden globally, especially prevalent in Asian and European regions. Despite a notable decline in incidence in the United States and Western Europe over recent decades, the disease's persistence underscores the urgency for advanced research in its pathogenesis and treatment strategies. Central to this pursuit is the exploration of the mitogen-activated protein kinase (MAPK) pathway, a pivotal cellular mechanism implicated in the complex processes of gastric cancer development, including cellular proliferation, invasion, migration, and metastasis.

Background And Objectives: Stroke-associated pneumonia (SAP) is the aftermath of aspiration of oropharyngeal secretions or stomach content. Mechanical ventilation and lowered immunity and consciousness facilitate the etiopathogenesis of SAP. Antibiotic prophylaxis and repeated culture and sensitivity testing dampen the drug susceptibility patterns of the pathogens.

Background: Preoperative fasting aims to reduce the risk of pulmonary aspiration. Our aim was to compare the incidence of increased gastric content after preoperative liberal versus a standard fasting in children.

Method: Two hundred children, presented for elective surgeries, were instructed to follow either 6-4-2 (standard group) or 6-4-0 (liberal group) preoperative fasting regimens.
