Background: Severe primary mitral regurgitation [degenerative MR (DMR)] is associated with poor outcomes, including cardiac death and hospitalization due to worsening heart failure. However, little information is available regarding the characteristics of moderate DMR and their impacts on prognostic outcome. The aim of the present study was to investigate the prognosis and its determinants in patients with moderate DMR.

Methods: We retroactively reviewed 13,700 consecutive patients who underwent transthoracic echocardiography and selected those with moderate DMR but without other underlying cardiac diseases. We assessed the characteristics and event-free rate of patients with moderate DMR compared with those of age- and gender-matched patients with mild or no MR.

Results: The cohort included 182 (1%) patients with moderate DMR, and these were compared with 182 age- and gender-matched patients with mild or no MR. During the follow-up period of 1376 ± 652 days, 30 patients (8%) met the composite endpoint defined as cardiac death or hospitalization due to worsening heart failure. Kaplan-Meier analysis showed that patients with moderate DMR were significantly associated with a poor outcome compared to patients with mild or no MR (log-rank test p < 0.0001). Cox proportional hazard ratio revealed that moderate DMR and paroxysmal atrial fibrillation (PAF) were the independent predictors of the composite endpoint.

Conclusions: Patients with moderate DMR and concomitant PAF had a significantly worse outcome compared to those with mild or no MR. Active surveillance and some intervention for patients with PAF and moderate DMR may be required.

Background: Mitral valve transcatheter edge-to-edge repair (M-TEER) was initially indicated for central degenerative mitral regurgitation (DMR) lesions, but advancements in technology have enabled successful treatment in an increasing number of noncentral DMR patients.

Aims: This study aims to compare procedural outcomes and prognosis between noncentral DMR patients, outside clinical trial anatomical criteria, and central DMR patients undergoing M-TEER.

Methods: Drug-refractory moderate-to-severe DMR patients treated with M-TEER at Fuwai Hospital from January 2021 to February 2024 were retrospectively analyzed.

Article Synopsis
  • Scientists are looking for more chemically complex hydrides that can achieve high superconductivity at lower pressures, beyond just hydrogen-rich materials.
  • They simplify the task of studying these complex structures by using electronic indicators related to superconductivity, rather than complex calculations.
  • The research proposes three classes of hydrides with strong hydrogen connectivity, each designed to potentially enhance superconductivity based on their structural characteristics and electronic properties.

Article Synopsis
  • - Longitudinal electronic health records (EHR) are being analyzed using advanced unsupervised machine learning algorithms to uncover patterns in disease development and progression, particularly in real-world clinical settings.
  • - A study tested 30 state-of-the-art algorithms on simulated clinical data, finding that dynamic time warping techniques performed best in accurately classifying clinical datasets, particularly those with varying blood pressure measurements.
  • - The most effective algorithm was applied to cluster a large dataset of pediatric metabolic syndrome cases, revealing distinct patterns of childhood BMI, among which a consistently high BMI cluster showed the highest risk for metabolic syndrome, aligning with existing research findings.

The prevalence, etiologies and clinical consequences of tricuspid regurgitation associated with severe degenerative mitral regurgitation.

Eur Heart J Cardiovasc Imaging

August 2024

Cardiovascular Imaging Unit, Cardiothoracic Department, IRCCS San Raffaele Scientific Institue, Milan, Italy.

Aims: The prevalence, the etiologies and the clinical features of tricuspid regurgitation (TR) in the context of concomitant degenerative mitral valve (MV) disease are poorly defined. This paper aims to assess the prevalence, determinants and clinical consequences of TR in severe degenerative mitral regurgitation (DMR).

Methods And Results: Clinical and echocardiographic characteristics were collected among patients with severe DMR.

Chloroformate-mediated ring cleavage of indole alkaloids leads to re-engineered antiplasmodial agents.

Natural product ring distortion strategies have enabled rapid access to unique libraries of stereochemically complex compounds to explore new chemical space and increase our understanding of biological processes related to human disease. Herein is described the development of a ring-cleavage strategy using the indole alkaloids yohimbine, apovincamine, vinburnine, and reserpine that were reacted with a diversity of chloroformates paired with various alcohol/thiol nucleophiles to enable the rapid synthesis of 47 novel small molecules. Ring cleavage reactions of yohimbine and reserpine produced two diastereomeric products in moderate to excellent yields, whereas apovincamine and vinburnine produced a single diastereomeric product in significantly lower yields.
