Ileal Neuroendocrin Tumor Metastasis to Breast Diagnosed with Ga-68 DOTATATE PET/CT.

Breast metastasis of the well differentiated neuroendocrin tumor (WDNET) of the ileum is very rare. A case of a 62-year-old woman with ileal WDNET, who underwent restaging with Ga-68 DOTATATE PET/CT due to progression of metastatic lesions under the treatment with somatostatin analog and mammalian target of rapamycin inhibitors. Ga-68 DOTATATE PET/CT demonstrated intense increased uptake in the subsantimetric nodular lesion in the upper outer quadrant of the left breast. The histopathologic findings obtained by tru-cut biopsy revealed WDNET metastasis (Ki-67 proliferation index 1%).

We present the imaging findings of a 48-year-old woman that metastasized to multiple bones with a history of duodenal gangliocytic paraganglioma. 68Ga-DOTATATE PET/CT showed multiple osteolytic bone destruction with intense uptake. Multiple bone metastases originating from duodenal gangliocytic paraganglioma confirmed histopathological results of a biopsy on the chest-back bone.

Accidental intra-arterial injections of radiotracers are rare events resulting in a specific imaging pattern, described as "hot-forearm," "hot-hand," "glove-phenomenon," or "glove-like pattern." We present a case 68 Ga-DOTATATE total-body PET/CT for restaging of a neuroendocrine tumor, where intra-arterial misinjection resulted in a glove phenomenon. Since patients may present with minimal symptoms, like in this case, and PET findings may only be seen at the distal upper extremity (placed above the head), these accidental injections may be more frequently detected with total-body PET/CT due to the longer field-of-view.
