Melanoacanthoma is a benign epithelial tumor composed of melanocytes and keratinocytes that can morphologically mimic malignant neoplasms. Two patients with melanoacanthoma that clinically masqueraded as melanoma are described: a 65-year-old African-American woman with a pigmented nodule on the right preauricular area and an 85-year-old Haitian-Creole man with a large exophytic nodule on his left lower abdomen. Melanoma was clinically suspected in both patients. Biopsies were performed, which established the diagnosis of melanoacanthoma. Complete removal of a melanoacanthoma should be considered since partial excision may result in recurrence.

Article Synopsis
  • Basal cell carcinoma (BCC) is the most common type of primary skin cancer, with pigmented BCC being a rare variant that can resemble nodular melanoma (NM).
  • A case is presented involving a 40-something woman with a hyperpigmented nodule on her cheek that looked like pigmented NM, but dermoscopy revealed distinct features.
  • After initial biopsy showed signs of trichoepithelioma, complete excision confirmed it was pigmented BCC; awareness of this rare presentation is essential for proper diagnosis and management, especially in dark-skinned patients.

Malignant Melanoacanthoma.

Am J Dermatopathol

December 2023

Department of Dermatology, Kaiser Permanente Los Angeles Medical Center, Los Angeles, CA.

Melanoacanthomas are benign variations of seborrheic keratosis that have been known to mimic other common benign and malignant skin lesions. Therefore, the diagnosis typically requires biopsy and careful histologic examination. Here, we present the case of a 25-year-old woman initially diagnosed clinically with an epidermal inclusion cyst, but, on biopsy and further evaluation, was found to have histological features of an atypical or malignant melanoacanthoma.

Article Synopsis
  • * Histological findings included epithelial acanthosis and melanin incontinence, with TRP2-positive melanocytes found in all cases, while T-lymphocytes predominantly outnumbered B-lymphocytes.
  • * A broader scoping review identified 85 additional cases, predominantly in females and black-skinned individuals, suggesting that oral melanoacanthoma primarily affects the buccal mucosa and may arise from specific immune cell involvement.
