Case: We report a rare cause of patella baja in a 25-year-old man presenting with right knee pain. He was found to have patella baja and a genu recurvatum deformity. Imaging revealed an extremely unusual variant of the patellar tendon-identified to have an intramedullary tibial insertion. A patellar tendon reconstructive procedure was performed.
Conclusions: An intramedullary insertion of the patellar tendon is a fascinating and rare cause of patella baja with subsequent altered biomechanics of the extensor mechanism. We believe this is the first case report to describe its surgical presentation and an operative approach to its management.

In this case report we describe evaluating a patient for a traumatic knee arthrotomy using ultrasound in a resource-limited medical clinic at the base of a ski area. A 23-y-old female presented with a laceration superior to the patella of the left leg. On examination, the wound tracked deep, and providers had concern for traumatic arthrotomy.

The human patellar tendon contains distinct fascicle bundles across its mediolateral and anteroposterior regions. Studies have suggested region-specific behaviour during in vivo actions, but it is unclear whether such regional differences result from localized variation in composition and mechanical properties within the tendon itself. Furthermore, the viscoelastic properties of any region of the human patellar tendon have not been well described previously.

Introduction: Mobile-bearing (MB) inserts, designed to minimize aseptic loosening and to reduce contact stresses leading to polyethylene wear, are an alternative to fixed-bearing (FB) inserts. Most studies have shown no significant difference between MB and FB constructs, and there is limited long-term data comparing the two constructs [1,2,3,4].
