Evaluation of a sustained-release buccal nitroglycerin preparation in stable angina pectoris.

Int J Clin Pharmacol Res

Division of Cardiac Rehabilitation, Civil Hospital of Brescia, Fasano del Garda, Italy.

Published: July 1988

The efficacy of a 2.5 mg sustained-release buccal nitroglycerin preparation given 8-hourly in effort-induced stable angina pectoris was investigated by means of graded exercise testing in 15 patients. An initial double-blind crossover study, compared with a placebo (duration six days) was followed by a further 15-day open treatment period on the active drug. Exercise testing was carried out 2 and 7 h after tablet administration at the conclusion of active treatment in the double-blind phase, and 2 h after tablet administration at the conclusion of the open treatment phase. Workload and exercise duration were significantly increased and mean electrocardiographic ST segment depression and ST segment recovery time significantly reduced by buccal nitroglycerin in the initial phase of the study, and these improvements were maintained over the subsequent 15-day assessment period. Systolic blood pressure at rest was significantly decreased by active treatment; other haemodynamic parameters remained unchanged. The study demonstrated the efficacy of sustained-release buccal nitroglycerin in effort-induced stable angina pectoris, and an absence of (+) tolerance to the therapeutic effects of this mode of nitrate administration.

Salvaging the dehisced glans penis.

J Pediatr Urol

October 2018

RWJ Saint Barnabas Medical Center, West Orange, NJ, USA; New York-Presbyterian Weill Cornell Hospital, New York, USA.

Objective: The glans penis may show a deep groove (surgically favorable), or may appear flat with an absent sulcus (unfavorable). Glans dehiscence following hypospadias repair, especially after multiple surgeries, frequently results in a scarred, obliterated, or absent urethral plate. The glans penis appears to be flat and grooveless.

The Role of Nitroglycerin and Other Nitrogen Oxides in Cardiovascular Therapeutics.

J Am Coll Cardiol

November 2017

Department of Medicine, Brigham and Women's Hospital, Harvard Medical School, Boston, Massachusetts. Electronic address:

The use of nitroglycerin in the treatment of angina pectoris began not long after its original synthesis in 1847. Since then, the discovery of nitric oxide as a biological effector and better understanding of its roles in vasodilation, cell permeability, platelet function, inflammation, and other vascular processes have advanced our knowledge of the hemodynamic (mostly mediated through vasodilation of capacitance and conductance arteries) and nonhemodynamic effects of organic nitrate therapy, via both nitric oxide-dependent and -independent mechanisms. Nitrates are rapidly absorbed from mucous membranes, the gastrointestinal tract, and the skin; thus, nitroglycerin is available in a number of preparations for delivery via several routes: oral tablets, sublingual tablets, buccal tablets, sublingual spray, transdermal ointment, and transdermal patch, as well as intravenous formulations.

Objective: To evaluate the clinical efficacy of acupuncture combined single photon emission computed tomography (SPECT) in treating coronary heart disease (CHD) and its effect on the myocardial ischemia/perfusion and the recovery of heart functions.

Methods: Totally fifty-nine patients with confirmed CHD were randomly assigned to two groups, the acupuncture group (32 cases) and the nitroglycerine group (27 cases). Patients in the acupuncture group were electro-acupunctured at bilateral Neiguan (PC6) and Xinshu (BL15) for 30 min with the frequency of 2/15 Hz and the current strength 9 - 18 mA after myocardial imaging induced by routine exercises or drug load.

Delivery of insulin to the buccal mucosa utilizing the RapidMist system.

Expert Opin Drug Deliv

September 2008

Albert Einstein College of Medicine, 48 Carleon Avenue, Larchmont, NY 10538, USA.

Background: The burgeoning number of people with diabetes mellitus is a global problem. Simple but effective treatment with minimal side effects will be required to reduce the risk for macro- and micro-vascular complications. A big part of this goal can be achieved by the ready administration of insulin with or without other medications.

By using NADPH-diaphorase (NADPH-d) histochemistry, nitric oxide synthase (NOS) immunohistochemistry, Western blotting, and NO pharmacology, we investigated the distribution and possible function of NOS-containing neurons in different units of the alimentary tract of the snail, Helix pomatia. Discrete populations of neurons in the buccal ganglia displayed NADPH-d reactivity. NADPH-d-reactive and NOS-immunoreactive (NOS-IR) neurons were present in the caecum, and labeled fibers were found to innervate the circular muscles of the proesophagus and caecum and to form axosomatic connections with neurons of the myenteric and submucosal plexi of the caecum.
