The optimum antithrombotic strategy before open heart surgery in patients with recently implanted coronary stents has not been clearly established because of lack of clinical evidence. This report describes a case of stent thrombosis in a patient who discontinued a P2Y inhibitor (ticagrelor) for 3 days before coronary artery bypass grafting. The period of withdrawal of antiplatelet drugs before coronary artery bypass grafting should be minimized in patients with recently implanted drug-eluting coronary stents to prevent stent thrombosis.
