The Utility of Thromboelastography to Guide Blood Product Transfusion.

Am J Clin Pathol

Department of Pathology and Laboratory Medicine, University of Rochester Medical Center, Rochester, NY.

Published: September 2019

Objectives: To provide an overview of the clot viscoelastic testing technology and to describe its utility in guiding blood product transfusions.

Methods: A case scenario will be discussed as well as interpretation of thromboelastography (TEG) tracings. In addition, literature examining the utility of viscoelastic testing in guiding patient management and blood product transfusions will be reviewed.

Results: TEG/rotational thromboelastometry (ROTEM) is useful in evaluating clot kinetics in trauma and acutely bleeding patients. TEG/ROTEM parameters are reflective of values measured using standard coagulation assays; however, TEG/ROTEM parameters are more rapidly available and more costly. TEG and ROTEM are used in three main settings: cardiac surgery, liver transplantation, and trauma to assess global hemostasis and administration of blood products.

Conclusions: TEG/ROTEM can be helpful in guiding resuscitation and blood product transfusion. Several studies have demonstrated a reduction in transfusion of blood components with TEG/ROTEM; however, other studies have suggested that TEG/ROTEM is not clinically effective in guiding transfusion.

Background: Beta-lactams remain the first-line treatment of infections despite the increasing global prevalence of penicillin-resistant/non-susceptible strains. We conducted a cross-sectional household survey in a rural community in northern Vietnam in 2018-2019 to provide prevalence estimates of penicillin non-susceptible (PNSP) carriage and to investigate behavioural and environmental factors associated with PNSP colonization. The data presented will inform the design of a large trial of population-based interventions targeting inappropriate antibiotic use.

Investigating BoLA Class II DRB3*009:02 carrying cattle in Japan.

Enzootic bovine leukosis (EBL) is a malignant lymphoma of cattle that is mainly caused by bovine leukemia virus (BLV) infection. In this study, PCR-RFLP was used to investigate the frequency of the DRB3*009:02 allele in several farms with different herd management practices in Japan. A total of 742 Holsteins (384) and Japanese Blacks (230) were used as the sample size for the study, which was larger than the number of cattle in the study area with a confidence level of 95 % and a margin of error of 8.

Introduction: Recurrent uveitis (RU), an autoimmune disease, is a leading cause of ocular detriment in humans and horses. Equine and human RU share many similarities including spontaneous disease and aberrant cytokine signaling. Reduced levels of SOCS1, a critical regulator of cytokine signaling, is associated with several autoimmune diseases.

Purpose: Mitoxantrone (MTX) is largely restricted in clinical usage due to its significant cardiotoxicity. Multiple studies have shown that an imbalance in the gut-heart axis plays an important role in the development of cardiovascular disease (CVD). We aim to explore the possible correlations between gut microbiota (GM) compositions and cardiometabolic (CM) disorder in MTX-triggered cardiotoxicity mice.

Because acute kidney injuries (AKI) are one of the critical health problems worldwide, studies on the risk factors, mechanisms, and treatment strategies seem necessary. Glycerol (GLY), known to induce cell necrosis via myoglobin accumulation in renal tubules, is widely used as an AKI model. This study aimed to evaluate the protective effects of gallic acid (GA) against GLY-induced AKI.
