Background: Lipomatous meningiomas are a rare variety of meningioma, in which fat storage occurs in adipocytes and lipoblasts are found within the tumor. World Health Organization grade II atypical meningiomas with lipometaplasia and extensive hyperostosis of calvarium is a rare type with a good prognosis after complete removal.
Case Description: Here we report a case of intracranial atypical meningioma with lipometaplasia and disproportionately large hyperostosis of overlying calvarium in a 40-year-old man. His computed tomography and magnetic resonance imaging scans showed right frontotemporoparietal homogenous bony swelling with an underlying diffusely enhancing extra-axial, dural-based lesion with areas of hyperintensity on T1 and isointensity on T2 with perilesional edema. Microscopically, it revealed a meningeal neoplasm with hypercellularity, small cell changes, sheathing pattern, and extensive lipomatous metaplasia.
Conclusions: World Health Organization grade II atypical meningiomas with lipometaplasia and extensive hyperostosis of calvarium is a rare type with a good prognosis after complete removal. Clinical features in patients are similar to those of conventional meningioma; however, radiologic features depend on the amount of fat present in the tumor. To the best of our knowledge, there are no case reports in the current literature regarding this. Every new case will widen our horizon. Meningothelial cells exhibit lipomatous changes as a result of metabolic abnormalities.

Pleomorphic adenoma (PA) is a commonly occurring benign salivary gland tumour known for its surfeit of histo-morphologic appearances ranging from myxochondroid, plasmacytoid, and squamoid elements to the infrequently seen osseous and adipose tissue metaplasia. Oncocytic metaplastic changes, a rare finding, present in the form of sheets/islands/cords of polygonal cells with rich eosinophilic or occasionally clear cytoplasm and an oval/round nucleus. We report one such rare case which portrayed extensive oncocytic changes in a unique bilayered/multi-layered epithelium, lining the papillary cystic areas similar to that of Warthin's tumour and extensive keratinisation simulating the newly recognised entity "adnexa-like differentiation/trichilemmal keratinisation", along with foci of cholesterol clefts and multi-nucleated giant cells.

Spindle cell myoepithelioma with extensive lipomatous metaplasia and thick collagen bundles has not yet been described, and there are no published reports on its cytological appearance in fine-needle aspiration (FNA). A 49-year-old man presented with a painless mass in the right submandibular area that had been gradually enlarging for a period of 5 years. The cytologic smears showed fascicles of cohesive spindle cells as well as individual bland cells with bipolar naked nuclei in a fibrillary background.

Fat-containing tumors of the salivary glands are uncommon. Their wide histological spectrum varies from pure lipomatous neoplasms similar to their cutaneous and soft tissue counterparts to mixed lipoepithelial lesions specific to the salivary glands. With few exceptions, these uncommon lesions affect mainly the elderly, with a mean age at presentation of ≥ 50 years and show a predilection for males.
