AI Article Synopsis

  • Pembrolizumab is an antibody used in cancer immunotherapy that specifically targets the PD-1 receptor on T cells, but it can cause immune-related side effects.
  • The report describes a unique case of mucous membrane pemphigoid affecting a patient with ovarian adenocarcinoma, manifesting in areas like the mouth and eyes but not the skin, and it continued after stopping the drug.
  • The text also includes a review of other cases related to pembrolizumab that involve bullous pemphigoid and discusses potential mechanisms behind these skin lesions.

Article Abstract

Pembrolizumab is a humanized antibody that targets the programmed death-1 receptor expressed in T cells with high selectivity. This therapeutic is of great importance in cancer immunotherapy yet managing the potential immune-related adverse events remains a concern. Here, we report a rare case of mucous membrane pemphigoid in the oral mucosa, upper respiratory tract, and conjunctiva of a patient with ovarian adenocarcinoma without cutaneous manifestation, which persisted even after pembrolizumab discontinuation. A brief review of pembrolizumab-related bullous pemphigoid cases is presented and possible mechanisms underlying these lesions are discussed.

Drug nanocrystal engineering is an attractive pharmaceutical approach to enhancing the oral bioavailability of poorly soluble drugs. The mechanism of drug nanocrystal stabilization, however, is unclear. Here we developed andrographolide nanocrystals (AG-NCs) with various nonionic surfactants (Pluronic-F127, TPGS, or Brij-S20).

Bullous pemphigoid (BP) is the most prevalent autoimmune subepidermal blistering disease of the skin and mucous membranes. This disease typically affects the elderly and manifests with pruritus and localized or, most commonly, generalized bullous lesions. Numerous studies have established the association between BP and oral antidiabetic agents, particularly dipeptidyl peptidase 4 (DPP4) inhibitors, diuretics, and certain antibiotics, notably levofloxacin and cephalexin.

Mucinous carcinoma is a rare clinical disease. Although well described in the literature, a mucinous carcinoma diagnosis is often difficult due to its atypical clinical presentation. We report a female patient with a right inguinal mass and ileocecal myxo carcinoma who was misdiagnosed as having a right incarcerated inguinal hernia invading the peritoneum incarcerated inguinal hernia and ileocecal myxo carcinoma.

We present the case of a 36-year-old paraplegic woman with a history of spinal cord injury who developed a generalized blistering rash, later diagnosed as bullous pemphigoid (BP). During her hospitalization, she was treated with prednisone and rituximab infusions, transitioning to maintenance therapy with topical steroids, doxycycline, and nicotinamide. A year later, she presented with concerns about a BP flare on her feet.
