Extensor pollicis longus rerouting is a common procedure to improve thumb abduction in thumb-in-palm deformity seen with spastic cerebral palsy. In 1985, Manske redirected the extensor pollicis longus tendon in this procedure through the first extensor compartment. They also proposed an alternative subcutaneous route around the extensor pollicis brevis and abductor pollicis longus tendons proximal to the extensor compartment. In this study, we performed a three-dimensional analysis of thumb motion on 11 cadaver arms with the subcutaneous route and the first extensor compartment route. We found that with the two different routing methods, the mean difference in thumb radial abduction and retropulsion (0° or 6°, respectively) was very small. Such differences are unlikely to have clinical relevance. We were unable to find significant differences in the motion range of the thumb after these rerouting techniques or sites of insertion. Our biomechanical data support the simpler subcutaneous route.

Here we report the long-term outcomes of novel, individualized reconstruction strategies using combined nerve and tendon transfer procedures (CNaTT) to restore prehension and grasp in two patients with tetraplegia.
Case Presentation: Two women, 45 years of age, underwent bilateral nerve transfer according to the Bertelli S-PIN (supinator to posterior interosseous nerve transfer) procedure.

Introduction: Conservative treatment options, such as rest, massage, cold and heat packs, wrist splints, braces, physical therapy, thumb spica casts, and local steroid injections, have been used with mixed results to treat De Quervain's tenosynovitis. Surgical treatment, like releasing the first dorsal wrist compartment, is the last resort for resistant cases of De Quervain's tenosynovitis, exhibiting an efficacy of 91%. However, complications and increased expenses have limited the use of surgical interventions.

Clasped thumb is characterised by thumb adduction and flexion deformity at the metacarpophalangeal joint caused by abnormalities in the extensor tendons. While conservative treatments are effective in infants, surgical intervention is necessary for older who experience deformities or functional impairments. We report an adolescent patient with an untreated clasped thumb who underwent extensor pollicis brevis (EPB) reconstruction using the extensor indicis proprius (EIP) tendon through the first dorsal compartment.
