The occipital artery (OA) can be used as a donor artery in many types of bypasses; however, harvesting this artery in the depths of its course is challenging. In order to safely harvest the OA, a large hockey-stick-shaped incision is often used, even when only a retrosigmoid approach is needed for the intracranial procedure. In this article, we describe a detailed technique whereby the OA is harvested via a suboccipital subperiosteal-transperiosteal dissection technique. In this technique, the occipital groove is identified via a small linear incision. With the technique described, the OA can be localized and harvested effectively for bypass in the subsequent retrosigmoid approach.

Background: Petroclival meningiomas are still a neurosurgical challenge due to their proximity to cranial nerves and cerebral vasculature along the surgical corridor. The usual extension of large petroclival meningiomas is along the posterior fossa, frequently compromising and displacing adjunct cranial nerves such as the sixth and seventh-eight cranial nerve complex with brainstem compression, causing progressive neurological deficit and severe headache. The goal of sizeable petroclival meningioma surgery treatment is a maximal resection with preservation of neurological function.

Methods: Following PRISMA guidelines, a systematic review was conducted by searching PubMed, EMBASE, Web of Science, and Cochrane databases from inception to 19 June 2024. Studies regarding adult patients with histologically confirmed MB radiographically confirmed to be located in the CPA were included.

Background And Objectives: This article aims to report results of our facial nerve preservation approach to treating vestibular schwannomas (VS) at a single institution by a single surgeon performing both microsurgery (MS) and stereotactic radiosurgery (SRS).
Methods: We retrospectively reviewed 751 patients at our institution between 1998 and 2023 by intervention received: retrosigmoid microsurgery (MS, Group 1, 217 patients), gamma knife stereotactic radiosurgery (SRS, Group 2, 462 patients), MS then SRS (Group 3, 72 patients), SRS then MS (Group 4, 10 patients), and SRS then SRS (Group 5, 5 patients). No patients had MS followed by MS.
