Background: In patients with only upper (C5, C6) brachial plexus palsy (BPP), the pooled international data strongly favor nerve transfers over nerve grafts. In patients with complete BPP, some authors favor nerve grafts for the restoration of priority functions whenever there is a viable proximal stump.

Objective: To evaluate functional recovery in cases of upper and complete BPP where only direct graft repair from viable proximal stumps was performed.

Methods: The study included 36 patients (24 with complete BPP and 12 with only upper BPP) operated on over a 15-yr period. In all cases, direct graft repair from C5 to the musculocutaneous and the axillary nerve was performed. In cases with complete BPP, additional procedures included either direct graft repair from C6 to the radial nerve and the medial pectoral nerve or the dorsal scapular nerve transfer to the branch for the long head of the triceps.

Results: The use of C5 proximal stump grafts (in both complete and upper BPP) resulted in satisfactory elbow flexion in 26 patients (72.2%) and satisfactory shoulder abduction in 22 patients (61.1%). The use of C6 proximal stump grafts in patients with complete BPP resulted in satisfactory elbow extension in 5 (50%) and satisfactory shoulder adduction in another 5 (50%) patients.

Conclusion: Although nerve transfers generally enable better restoration of priority functions, in cases of infraganglionary injuries, especially in shorter defects, it is also necessary to consider direct graft repair, or at least its combination with nerve transfers, as a potentially beneficial treatment modality.

Purpose: The purpose of this study is to develop an evidence-based algorithm for the management of symptomatic bipartite patella in the pediatric and adolescent population based on a systemic review of the published literature.

Methods: A systematic review of the literature was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines on PubMed and Embase, selecting for studies discussing the management of symptomatic bipartite patella.

Results: Five studies met criteria, involving 315 knees (314 patients, average age 15.

Best practices guidelines in the postoperative management of patients who underwent cervical and lumbar fusions.

J Spine Surg

September 2024

Department of Neurosurgery, New York Brain & Spine Surgery, West Harrison, NY, USA.

Background: There is a lack of standardization in spine surgery in particular, as relates to postoperative care for the most common spine procedures such as cervical and lumbar fusions. The goal of this study was to develop a standardized postoperative treatment protocol for common spine procedures such as cervical and lumbar fusion to reduce unnecessary visits, imaging studies, and create a standard for all spine surgeons to adhere while maintaining quality.

Methods: We developed a best practices protocol (BPP) for postoperative spine care for anterior cervical diskectomy and fusion (ACDF) and posterior lumbar interbody fusion (PLIF).

This study was designed to determine how impulsivity, self-regulation, and response withholding are related to one another and to university students' drinking behavior. Participants ( = 108) completed measures of impulsivity, self-regulation, and alcohol consumption. In addition, a computerized Go/No Go task and a backward memory task were used to measure participants' behavioral impulsivity and their memory capacity.

: this study aimed to examine the impact of interaction between body posture and the quality of movement patterns on injury frequencies in amateur athletes. : The study sample consisted of 89 young amateur athletes. Movement pattern quality was assessed by the Functional Movement Screen (FMS), test and body posture in the frontal plane was assessed by the moire method for the parameters Shoulder Slope Angle, Lower Scapula Protrusion Difference, and Pelvic Tilt Angle.

[Fe(bpp)][ClO] (bpp = 2,6-bis{pyrazol-1-yl}pyridine; monoclinic, 2/) is high-spin between 5-300 K, and crystallises with a highly distorted molecular geometry that lies along the octahedral-trigonal prismatic distortion pathway. In contrast, [Ni(bpp)][ClO] (monoclinic, 2) adopts a more regular, near-octahedral coordination geometry. Gas phase DFT minimisations (ω-B97X-D/6-311G**) of [M(bpp)] complexes show the energy penalty associated with that coordination geometry distortion runs as M = Fe (HS) ≈ Mn (HS) < Zn ≈ Co (HS) ≲ Cu ≪ Ni ≪ Ru (LS; HS = high-spin, LS = low-spin).
