Mammary duct ectasia (MDE) is a benign and often troublesome breast condition. The usual presentation includes colored nipple discharge and mastalgia. Nipple hypersensitivity has never before been described as a presenting complaint for MDE. This case report looks into such an unusual case.

Mass-forming type 2 autoimmune pancreatitis with upstream dilatation of the main pancreatic duct dilatation: a case report.

J Med Case Rep

Department of Surgery 1, University of Fukui, 23-3 Matsuokashimoaizuki, Eiheiji-Cho, Yoshida-gun, Fukui, 910-1193, Japan.

Background: Type 2 autoimmune pancreatitis is characterized by multiple or segmental strictures of the main pancreatic duct without upstream dilatation. We encountered a case of mass-forming type 2 autoimmune pancreatitis with upstream main pancreatic duct dilatation that was difficult to diagnose preoperatively using endoscopic ultrasound sonography-guided fine-needle aspiration cytology.

Case Presentation: A 58-year-old Japanese man presented with recurrent acute pancreatitis secondary to a 10-mm pancreatic head tumor.

Article Synopsis
  • Autoimmune diseases can cause rare breast issues, often presenting as lumps or breast pain, and some patients may not show any symptoms at all.
  • The imaging characteristics of autoimmune mastitis are usually nonspecific, often appearing as irregular masses on ultrasounds or segmental enhancements on MRIs, which can make diagnosis difficult.
  • The text highlights a unique case of mastitis linked to Behcet's syndrome, discussing the rapid changes seen in imaging, and reviews existing literature on the radiological and histopathological aspects of autoimmune mastitis.

As an alternative instead of a repeat surgical intervention, results (feasibility, safety, and technical and clinical success rate) of EUS-guided pancreatic duct drainage (EUS-PD) in a consecutive patient cohort because of symptomatic postoperative anastomotic stenosis as part of a unicenter observational study in daily clinical practice are presented.EUS-guided puncture (19-G needle) of the pancreatic duct, pancreaticography, and advancement of a guide wire (0.035 inches) via the anastomosis into the small intestine after previous dilatation of the transgastric access site (using ring knife): 1.

Article Synopsis
  • The study investigates the challenges in diagnosing and treating unexplained dilation of the cholangiopancreatic duct (UDCD) and assesses the effectiveness of transduodenal ampullectomy (TDA) as a potential solution.
  • A retrospective analysis of 203 patients (14 from the hospital and 189 from other studies) showed that TDA enhances the diagnostic capability compared to traditional methods, with a significantly higher rate of identifying issues during surgery (78.41% vs. 65.94%).
  • The study concludes that TDA is a safe and effective option for diagnosing and treating UDCD, with varying survival rates based on the pathology found, and reports a complication rate of 27.07% but

Article Synopsis
  • The study aimed to determine if a dilatation of the common bile duct (CBD) can be seen in postmortem CT scans of individuals who died from opiate/opioid intoxication and if this dilatation can also be measured for other drug groups.* -
  • Researchers analyzed 125 cases of lethal intoxication using postmortem CT and compared them to 88 control cases without toxic substances, finding a significant difference in CBD diameters between the intoxication group and controls.* -
  • The results showed that only opiate and opioid cases had a significantly wider CBD, suggesting dilatation may indicate lethal opioid intoxication, but this should be considered alongside other evidence in postmortem investigations.*
