Reported here is a rare respiratory complication in a young primiparous woman in a remote community hospital in the Northern Territory, Australia. A Caucasian primiparous woman aged 23 years presented with chest pain following a very prolonged second stage of labour. On clinical examination a crunching and crackling sound was heard over the precordium, synchronous with the heartbeat (Hamman's sign). A chest radiograph confirmed the presence of pneumomediastinum. Complete resolution of symptoms and signs was achieved with supportive management.

Aims: This study aimed to investigate the predictive value of the platelet indices for the diagnosis of acute deep vein thrombosis (DVT) in pregnant women.

Materials And Methods: Pregnant women aged 20-45 years who presented to the emergency room with leg pain and edema and underwent bilateral lower extremity venous Doppler examination for DVT were included in the study. Platelet large cell ratio (P-LCR), platelet distribution width (PDW) and mean platelet volume (MPV) values, which provide information on current platelet activity in the hemogram parameters recorded at first presentation in the emergency department, were analyzed between the two groups.

Background: Black, Indigenous, People of Color (BIPOC) in medicine and women faculty have lower 10-year promotion rates than their White and male peers, despite controlling for productivity metrics. Promotion standards vary across institutions, but there is likely a common need to improve transparency and consistency while mitigating bias, inequity, and the harm of additional equity work that is commonly expected of Black, Indigenous, People of Color and women faculty (the so-called minority tax).

Methods: A promotion advisory committee consisting of clinical and research faculty at all ranks specified expectations for a faculty member at the associate or full professor ranks, with 10-15 examples given for each "mission" (clinical, research, and education).
