Presbyphonia is defined as aging of the voice, and its growing attention as a medical concern parallels the continuing increase of the geriatric population worldwide. It results from physiologic changes to several laryngeal structures, including the musculature, surrounding cartilage, and lamina propria of the vocal folds. Characterized by glottic insufficiency, dysphonia in the elderly typically presents as a deterioration of voice quality, altered pitch and fundamental frequency, vocal fatigue, and strain. Fortunately, there are multiple treatment options that have proven successful in improving quality of life and restoring vocal stability for these patients.

Objectives: Injection laryngoplasty (IL) is commonly performed for glottic insufficiency and has historically been performed under general anesthesia via direct laryngoscopy (DL), with an increasing number of procedures being performed in the office setting via flexible laryngoscopy (FL). Existing literature regarding complications of IL primarily addresses immediate peri-procedural complications and adverse reactions to the injection material. This is the first study utilizing a large multi-institutional database comparing complications of IL performed via DL versus FL.

Approaches to regenerate vocal fold in glottic insufficiency remains to be a focus for exploration. This is attributed to the applications of cells or biological molecules alone result in fast degradation and inadequate for regeneration. Development of an injectable hydrogel for glottic insufficiency is challenging, as it needs to be non-cytotoxic, elastic yet possess good strength and easy to fabricate.

Objectives: Vocal fold injection augmentation with hyaluronic acid (HA) fillers is a common treatment for glottic insufficiency. While generally well-tolerated, rare inflammatory reactions can occur in 3% to 5% of patients. All reported reactions resolved after corticosteroid treatment, and no cases of recurrent inflammatory reaction with airway obstruction following steroid treatment have been reported.
