An 82-year-old man developed acute breathlessness and cyanosis, exacerbated while upright and improved on lying flat (platypnea-orthodeoxia syndrome). Echocardiography revealed acute torrential tricuspid regurgitation due to a flail posterior leaflet leading to right-to-left shunting through a patent foramen ovale (PFO). The patient's symptoms resolved after transcatheter PFO closure and tricuspid valve edge-to-edge repair.

Actinomycosis is a rare, chronic, and slow-progressing bacterial infection caused by . The condition's rarity, nonspecific clinical symptoms, and occasional occurrence in atypical locations often lead to delays in diagnosis, which are critical for initiating timely treatment and preventing further complications. We report the case of an 82-year-old man who initially developed pustules on the dorsum of his left hand, which gradually progressed over 15 years into an exophytic mass.

An 82-year-old man presented with acute respiratory distress, a one-week history of dry cough, and worsening dyspnea. Chest computed tomography revealed bilateral diffuse ground-glass opacities, raising suspicion of rapidly progressive interstitial pneumonia. Rapid autoantibody testing confirmed interstitial pneumonia with autoimmune features (IPAF), likely triggered by an upper respiratory infection.

Acute esophageal necrosis (AEN) is a rare phenomenon, usually presenting with hematemesis or melena in patients who are already unwell with concurrent pathology. We present a case of AEN in an acutely unwell 82-year-old man who presented in diabetic ketoacidosis (DKA). He was successfully managed in our rural centre with remote guidance from a tertiary centre.
