Immunologic examination was done in 26 patients on to 20 years after posttraumatic splenectomy at the age of 3 to 6 years of life. Immunologic parameters used were: concentration of IgG, IgA, IgM, isoagglutinins, heterophila antibodies, agglutinins against staphylococci E. coli and pertussis, serum opsonins for E. coli and pneumococci, serum haemolytic complement activity, C3C4, B- and T- subpopulations of lymphocytes, inhibition migration of mononnuclear cells with E. coli. Pathologic susceptibility to infections was not observed clinically. IgM, E. coli agglutinins and E. coli opsonins were significantly lower in the postsplenectomy group than in controls.

The management of postoperative pain in pediatric patients undergoing emergency surgical procedures, particularly in non-pediatric hospitals, presents significant challenges due to the unique physiological requirements of children. The utilization of opioid analgesia may result in severe complications, necessitating a transition toward multimodal analgesia, which integrates various pain management strategies to enhance effectiveness while mitigating adverse effects. Locoregional anesthesia techniques, such as fascial plane blocks, provide targeted pain alleviation, reducing dependence on opioids.

Background: Non-operative management for pediatric blunt splenic injury is well established in high-income countries, leading to a low splenectomy rate in hemodynamically stable children. Splenectomy rate became a quality indicator for Trauma Center verification utilized by the American College of Surgeons Committee on Trauma. However, data on splenectomy rate in children from countries with different income levels, such as Brazil, remain limited.

A Case of Traumatic Splenic Arteriovenous Fistula Treated by Transcatheter Arterial Embolization.

Cureus

July 2024

Department of Emergency Medicine and Critical Care, Saitama Medical Center, Saitama Medical University, Kawagoe, JPN.

Transcatheter arterial embolization (TAE) has increasingly replaced surgery for treating solid organ injuries, including the spleen, due to its minimally invasive approach. Studies show only a 3% splenectomy rate after TAE, despite a 10% incidence of missed vascular injuries in the American Association for the Surgery of Trauma (AAST) grade III splenic injuries on initial computed tomography (CT) scans. However, there's a lack of high-quality studies recommending specific CT follow-up intervals after non-operative management (NOM) of splenic injuries or guidelines for initiating treatment in cases of pseudoaneurysms or arteriovenous fistulas (AVFs).

Information on asplenic Lyme borreliosis (LB) patients with erythema migrans (EM) is lacking. We compared the course and outcome of 26 EM episodes in 24 post-trauma splenectomized patients (median age 51 years) diagnosed at a single clinical center in Slovenia during 1994-2023 with those of 52 age- and sex-matched patients with EM but with no history of splenectomy. All patients were followed for one year.

Monkeypox in a Kidney Transplant Recipient: Case Report and Literature Review.

Exp Clin Transplant

November 2023

From the Department of Nephrology, Dubai Hospital, Dubai Academic Health Corporation, Dubai, United Arab Emirates.

Monkeypox (mpox) infection is usually a self-limiting disease; however, kidney transplant recipients may be at a higher risk of serious complications, due to their immunosuppressed status. Nevertheless, the reported mpox cases in transplant recipients are very few, thus data on the clinical course and prognosis details of mpox in transplant recipients are scarce. Indeed, only 2 cases of mpox in kidney transplant recipients have been reported: one patient achieved good clinical recovery, and the other patient experienced a disseminated form of the disease with urinary and gastrointestinal complications.
