The impact of a tailored nutrition intervention delivered for the duration of hospitalisation on daily energy delivery for patients with critical illness (INTENT): a phase II randomised controlled trial.

Crit Care

January 2025

Australian and New Zealand Intensive Care Research Centre, School of Public Health and Preventive Medicine, Department of Epidemiology and Preventative Medicine, Monash University, 553 St Kilda Road, Melbourne, VIC, Australia.

Background: Nutrition interventions commenced in ICU and continued through to hospital discharge have not been definitively tested in critical care to date. To commence a program of research, we aimed to determine if a tailored nutrition intervention delivered for the duration of hospitalisation delivers more energy than usual care to patients initially admitted to the Intensive Care Unit (ICU).

Methods: A multicentre, unblinded, parallel-group, phase II trial was conducted in twenty-two hospitals in Australia and New Zealand.

Impact of mild hypercapnia on renal function after out-of-hospital cardiac arrest.

Resuscitation

December 2024

Australian and New Zealand Intensive Care Research Centre, Monash University, Melbourne, Australia; Department of Intensive Care, Austin Hospital, Heidelberg, Australia; Centre for Integrated Critical Care, The University of Melbourne, Melbourne, Australia.

Article Synopsis
  • Acute kidney injury (AKI) is a common and serious problem following out-of-hospital cardiac arrest (OHCA), particularly influenced by post-resuscitation cardiogenic shock (CS).
  • A study compared two groups of patients—those receiving targeted mild hypercapnia and those receiving targeted normocapnia—to see if higher carbon dioxide tension impacted AKI rates and other outcomes.
  • Results showed that approximately 72.1% of patients developed AKI regardless of treatment, with CS significantly increasing the likelihood of AKI, but carbon dioxide levels did not alter this relationship.

Objective: To assess the association of systolic and diastolic blood pressure (SBP and DBP) in recently menopausal women with white matter hyperintensity (WMH) volume later in life and determine whether short-term menopausal hormone therapy (mHT) modifies these associations.

Methods: Kronos Early Estrogen Prevention Study (KEEPS) was a multicenter, randomized, double-blinded, placebo-controlled 4-year mHT trial (oral conjugated equine estrogens or transdermal 17β-estradiol). KEEPS continuation was an observational follow-up of the participants 10 years after the end of mHT.

Background: Contingency management (CM), an incentive-based intervention to encourage target behaviors, effectively promotes medication adherence. However, efforts to extend CM to HIV pre-exposure prophylaxis (PrEP) have been lacking. As part of a randomized clinical trial to promote HIV Prevention among people who inject drugs (PWID), we examined the readiness of staff in community-based organizations serving PWID to implement CM for PrEP uptake and adherence in this population.

Background: Management of dorsal dead space and the aesthetics of a supratip break are paramount to achieving reproducible and reliable results in rhinoplasty.

Objectives: The authors present a modified technique of redraping the nasal soft tissue envelope in structural rhinoplasty by utilizing the nasal superficial musculoaponeurotic system (SMAS) to help obliterate dorsal dead space and restore normal anatomy, thereby enhancing midvault and supratip contouring.

Methods: A standard open rhinoplasty approach is utilized.
