Anterior mediastinal mass 4 years after type A aortic dissection.

A 47-year-old male who previously underwent emergency surgery for type A aortic dissection presented to the emergency department 4 years later with acute chest pain radiating to his back and a significant drop in hemoglobin. Clinical presentation was concerning for aortic graft failure, but imaging revealed a large (6.0 × 3.2 × 12.8 cm) soft tissue mediastinal mass that was not present 4 years before. Pathologic analysis revealed a rare thymic cyst with a hemorrhagic component. This was a challenging case from diagnostic and operative perspectives, involving the expertise of both cardiac and thoracic surgical teams.

Thoracic SMARCA4-deficient undifferentiated tumors (SMARCA4-UTs), recently recognized as a rare malignancy described in the 5th edition of the World Health Organization Classification of Tumors, are characterized by an inactivating mutation in SMARCA4, most commonly found in the mediastinum of male smokers. Despite the aggressive nature and poor prognosis associated with these tumors, which have a median survival time of approximately 4-7 months, no standardized treatment guidelines are currently established. There are currently no reported cases of extended progression-free survival (PFS) in SMARCA4-UT patients treated with surgery and immunotherapy.

Hodgkin's lymphoma (HL) is a malignancy of the lymphocytes in the lymph nodes and presents with non-specific systemic symptoms like fever, night sweats, and weight loss. While HL often involves the mediastinum, it rarely causes superior vena cava (SVC) syndrome, and eosinophilia is noted in approximately 15% of cases. Here, we report a unique presentation of HL in a 52-year-old male with a history of chronic pruritus, chronic kidney disease, and inactive hepatitis B.

[Foetal paraspinal neuroblastoma: A case report of autopsy findings].

Neuroblastoma is a rare tumour originating from neural crest cells, primarily occurring in the adrenal glands and sympathetic ganglia, with prenatal diagnosis often complicated by the difficulty in distinguishing it from other foetal abdominal or paraspinal masses. We present a case of foetal neuroblastoma in a 26-year old woman who, at 36 weeks of gestation, experienced absent foetal movements, leading to ultrasound confirmation of foetal demise with associated effusions. An emergency caesarean section revealed a stillborn male foetus with a previously undetected encapsulated mass in the posterior mediastinum, which was confirmed as neuroblastoma through histopathological analysis.

A 70-year-old man was referred to our hospital for an abnormal chest X-ray shadow. A chest CT scan showed a mass shadow with a cavitary lesion in segment 6 of the right lung. One year after radical resection(resection of the lower lobe of the right lung; pT2aN0M0, pStage ⅠB), the patient exhibited enlarged mediastinal lymph nodes and liver metastases, and postoperative recurrence was determined.
