Improvised Hydration Bladder Air Splint: A Wilderness Case Report.

Wilderness Environ Med

Published: March 2019

This case report describes the use of a hydration bladder as an improvised air splint for a left forearm fracture in an austere environment. The literature regarding the use of air splints in the prehospital and wilderness environments is scarce, and it does not appear that this technique has been previously described in the literature. Given the widespread use of hydration bladders, the adjustable nature and overall comfort of the splint, and the anecdotal success of this technique without any complications or harm to the patient, the authors believe this is an important improvised splinting option to add to the quiver of any outdoor enthusiast or wilderness medical professional.

Importance: Functional near-infrared spectroscopy (fNIRS) is a noninvasive technique used to quantify prefrontal cortex (PFC) neuroexcitation. The PFC is involved in the decision to void, and dysfunction in the region has been associated with overactive bladder (OAB). This study demonstrates neuroexcitation differences in the brain region associated with the decision to void (prefrontal cortex) using noninvasive fNIRS.

Bethanechol chloride (BTC), a quaternary ammonium compound used in bladder dysfunction treatment, requires challenges in developing optimal oral dosage forms due to its high water-solubility, short half-life, rapid elimination and four times a day administration. The aim of this study was to develop optimal BTC-loaded oral dosage forms that could provide both rapid onset and sustained therapeutic effects while reducing the frequency of conventional four-times-daily dosing (Mytonin® tablets). Four different BTC-loaded oral dosage forms were designed including gastro-retentive tablet (GRT), controlled-release tablet (CRT), bilayer tablet (BLT), and tablet-in-tablet (TIT).

Article Synopsis
  • Carboplatin is an effective and safer alternative to cisplatin for treating stage I seminoma, minimizing risks like nephrotoxicity and ototoxicity.
  • A case involved a 48-year-old man who experienced hematuria and acute kidney injury after receiving adjuvant carboplatin following surgery for testicular seminoma.
  • The acute kidney injury resolved after conservative management and the removal of a blood clot during cystoscopy, with imaging indicating potential ureteral obstruction.

Introduction: We sought to investigate the effects of pretreatment oral hydration on the outcomes of extracorporeal shockwave lithotripsy (ESWL).

Methods: Patients who undergoing ESWL for a single radio-opaque renal or proximal ureteric calculus ≤2 cm in size were randomized into two groups. The oral hydration group (OHG) administered 600 ml of water before ESWL, while the control group (CG) did not.

Positron emission tomography (PET) with F-FDG is being used more frequently to evaluate primary pelvic tumors (PTs). However, a standardized hydration protocol is essential for an optimal diuretic effect and constant results. We reviewed 109 patients with PTs who had undergone F-FDG PET/CT examinations between November 2006 and April 2013.
