In this work, we achieved the selective detection of wild and mutated rpoB gene in M. tuberculosis using an electrochemical DNA (E-DNA) sensor based on polypyrrole/FeO nanocomposite bearing redox naphthoquinone tag on PAMAM (spaNQ/PAMAM/PPy/FeO). The hybridization between a given probe and the complementary DNA target induced a large decrease in the naphthoquinone redox signal as measured by SWV and no cross-hybridization with single nucleotide mismatch DNA target occurred. Thanks to the catalytic properties of iron oxide nanoparticles combined with conducting properties of polypyrrole platform, we demonstrated that the transducing system allowed the detection of 1 fM of DNA target in a 50-µL drop corresponding to 3 × 10 copies of DNA. The sensor was able to detect the rpoB gene in PCR-amplified samples of genomic DNA and could also discriminate between the wild type rpoB gene and a single nucleotide mutated rpoB gene that provides resistance to rifampicin. Furthermore, the sensor could selectively detect the wild and mutant DNA in genomic samples without PCR amplification.

It was a general belief that drug resistance in Mycobacterium tuberculosis (Mtb) was associated with lesser virulence, particularly rifampicin resistance, which is usually produced by mutations in the RNA polymerase Beta subunit (RpoB). Interestingly, this kind of bacterial mutations affect gene transcription with significant effects on bacterial physiology and metabolism, affecting also the bacterial antigenic constitution that in consequence can produce diverse immune responses and disease outcome. In the present study, we show the results of the Mtb clinical isolate A96, which is resistant to rifampicin and when used to infect BALB/c mice showed hypervirulence, apparently by rapidly polarization of the Th2 immune response through early and high production of IL-4.

Rifampicin-resistant tuberculosis (RR-TB) is a critical issue with significant implications for patient care, public health, and TB control efforts that necessitate comprehensive strategies for detection. This study presents a novel point-of-care diagnostic tool for RR-TB detection employing a peptide nucleic acid (PNA)-paper-based sensor combined with isothermal recombinase polymerase amplification (RPA). The sensor targets mutations in codons 516, 526, and 531 of the rpoB gene, the top three common mutations associated with rifampicin-resistant strains.

Department of Medicine and Epidemiology, School of Veterinary Medicine, University of California-Davis, CA, USA.

Case Summary: A 9-year-old male castrated domestic shorthair cat from northern California was evaluated for a 12-month history of dermal and subcutaneous dermatitis in the inguinal region. Histopathologic examination of a biopsy revealed severe, chronic, multifocal to coalescing pyogranulomatous dermatitis and panniculitis, accompanied by ulceration and central necrosis. Aerobic bacterial culture of lesions yielded mycobacterial growth.

The identification and typing of bacteria are very expensive and time-consuming due to their growth times, and the expertise needed. MALDI-TOF MS represents a fast technique, reproducible with molecular approaches. This technique is still poorly applied in Legionella surveillance with estimation occurring only at the genus level.
