Case: We report the case of a pediatric terrible triad injury of the elbow consisting of an ulnohumeral dislocation, a radial neck fracture, and a coronoid chondral injury, which was diagnosed at 14 days following injury. We describe our surgical technique and provide details of the postoperative course, including the development of a rotational contracture following heterotopic ossification.

Conclusion: We describe a rare and challenging injury that should be recognized early and managed surgically to maintain acceptable elbow function.

The importance of parental involvement in the care and management decisions made for children hospitalized in the pediatric intensive care unit (PICU) is examined in this editorial. Initial days and weeks in a PICU can be challenging for the child and family due to the emotional intensity and medical complexity of the therapy a child receives. Regardless of the result, families may feel uncertain and anxious that their child may die or have a terrible outcome.

Chimeric antigen receptor T-cell (CAR-T) treatment has been widely used in the treatment of hematological malignancies, and its application has been gradually expanded to the research and treatment of solid tumors. However, unconventional types of response may occur after CAR-T treatment, such as hyperprogression, resulting in terrible outcomes. Here, we report the case of a 13-year-old adolescent boy with relapsed and refractory rhabdomyosarcoma who developed early hyperprogression 3 weeks after CAR-T treatment (target: B7H3 and CD171), which was detected by fluorine-18 fluorodeoxyglucose ( F-FDG) positron emission tomography (PET)/computed tomography (CT).

Article Synopsis
  • A terrible triad fracture of the elbow includes an elbow dislocation along with fractures of the coronoid process and radial head, which are rare in adults and even more so in children.
  • A case involving a 14-year-old boy was diagnosed through X-ray and confirmed with a CT scan after a closed reduction revealed an unstable elbow.
  • In surgery, the radial head and lateral epicondyle were fixed using K-wire, and the coronoid process was secured, leading to good clinical outcomes after two years, emphasizing the need for a CT scan post-reduction in such cases.

Background: Up to 20% of youth have impairing mental health problems as early as age 3. Early identification and intervention of mental health risks in pediatric primary care could mitigate this crisis via prevention prior to disease onset. The purpose of this study was to establish the feasibility and acceptability of implementing a brief transdiagnostic screening instrument in pediatric primary care for irritability and corollary impairment.

Methods: A retrospective analysis of CT scans was performed.
