Tofacitinib for refractory uveitis and scleritis.

Purpose: To report the successful use of tofacitinib in the treatment of refractory uveitis and scleritis.

Observations: Two patients, one with scleritis and another with anterior and intermediate uveitis, presented with refractory disease after failure of multiple steroid-sparing therapies. Treatment with tofacitinib led to durable resolution of uveitis and scleritis.

Conclusions And Importance: Tofacitinib is a potential novel treatment option for refractory, noninfectious inflammatory eye disease.

To report a case of spontaneous-onset unilateral scleritis and keratitis due to Pseudomonas aeruginosa in a patient with systemic lupus erythematosus who partially responded to antibiotic therapy and achieved complete resolution after IV immunotherapy. A 30-year-old woman with a past medical history of hypothyroidism and systemic lupus erythematosus and on irregular therapy presented with a long-history of thinning of the sclera and cornea refractory to antibiotic therapy despite a positive culture for P. aeruginosa, which eventually resolved with the implementation of immunotherapy.

Background: The application of biologic agents has benefited patients with Behcet's uveitis (BU) who do not respond to conventional treatment regimens. However, there is currently no consensus on the optimal treatment regimen of interferon alpha-2a (IFN-α2a) for refractory BU.
