Purpose: We describe a case of ureteral obstruction and fungal pyelonephritis in a cat. The case was managed successfully with a subcutaneous ureteral bypass (SUB) device and systemic antifungal medication.
Case Summary: A 9-year-old, male, neutered, domestic shorthair cat was referred for severe azotemia, ureteral obstruction, and sonographically identified progressive pyelectasia of the left kidney. A SUB device was placed. Cytology of the left kidney subsequently identified fungal organisms, and the cat responded clinically to treatment with fluconazole and supportive care. The cat survived for 11 months with intermittent fluconazole treatment.
Conclusion: Fungal pyelonephritis should be considered as a possible concurrent diagnosis in cats with ureteral obstruction. The successful clinical outcome in this case suggests that treatment with SUB placement, antifungal medication, and supportive care could be an effective treatment strategy.

The patients with Arthrogryposis-Renal dysfunction-Cholestasis (ARC) syndrome have genetic susceptibility to the opportunistic infections due to the involvement of VPS33B (vacuolar protein sorting 33 homolog B) in phagolysosome fusion in macrophages. Detailed pathologic studies in ARC patients are missing in literature due to the lack of autopsy. We described the first autopsy case of ARC syndrome in a 2-month-old male infant.

Objectives: The FOSFO-MIC study assessed the clinical and microbiological effectiveness, and safety of intravenous fosfomycin in treating complicated urinary tract infections (cUTIs) caused by Escherichia coli, in comparison with other intravenous antimicrobials.
Methods: A prospective, multinational matched-cohorts study involving adults with community-acquired cUTIs and receiving targeted therapy with intravenous fosfomycin or other first-line drugs (beta-lactams or fluoroquinolones) was conducted from November 2019 to May 2023 in 10 centres from Spain, Italy, and Türkiye. Matching criteria included healthcare-relation, Charlson and Pitt scores.
