Endovascular interventions are now used more frequently in the treatment of femoropopliteal occlusions, and antegrade passage through the occlusion is preferred as the first option. However, when antegrade passage fails for any of several reasons, retrograde, or less frequently, collateral passage may be used. Although collateral passage may present serious complications, it can be successfully applied in selected cases in experienced centers. In the present case, a superficial femoral artery occlusion was successfully opened with a collateral approach.

Background: Fractional flow reserve (FFR) can be estimated by analysis of intravascular imaging in a coronary artery; however, there are no data for estimated FFR in an extremity artery. The aim of this concept-generating study was to determine whether it is possible to estimate the value of peripheral FFR (PFFR) by intravascular ultrasound (IVUS) analysis also in femoropopliteal artery lesions.
Methods: Between April 2022 and February 2023, PFFR was measured before endovascular therapy in 31 stenotic femoropopliteal artery lesions.

A 13-year-old boy was referred to orthopedic surgery for chronic intermittent pain and swelling of the left knee. Initial imaging was consistent with osteochondritis dissecans of the femoral condyle. Follow-up imaging demonstrated unexpected progression, with a mass extending into the notch, replacing the anterior cruciate ligament, and eroding the femoral and tibial condyles.

Background: Bypass graft surgery is a key surgical intervention for ischemic heart disease (coronary bypass graft surgery) and critical limb ischemia (peripheral bypass graft surgery). Graft occlusion remains a significant clinical problem for both types. Further research into the pathobiological mechanisms of graft occlusion are needed in order to design targeted therapeutic strategies.
