  • Infective endocarditis (IE) can occur in adults with ventricular septal defects (VSD), and this case reports a rare instance involving systemic embolisms and aortic pseudoaneurysms.
  • A 27-year-old woman with a previously asymptomatic VSD presented with prolonged fever and was found to have staphylococcus infection, heart murmurs, and serious complications like mycotic aneurysms and embolisms.
  • The patient received targeted antibiotics and underwent urgent heart surgery, emphasizing the importance of checking for systemic embolism in right ventricle IE and noting that congenital VSD can lead to serious complications from bacteremia.

Mycotic intracranial aneurysms (MIAs) are rare but can cause significant morbidity and mortality due to rupture. Most patients have additional systemic medical comorbidities making endovascular treatment a vital modality in the treatment of these aneurysms. We aimed to share our institutional experience with the role of endovascular therapy in the treatment of mycotic aneurysms with a literature review.

SGLT-2 inhibitors (SGLT-2is) are considered to be a first-line treatment for common conditions like type 2 diabetes, chronic kidney disease, and heart failure due to their proven ability to reduce cardiovascular and renal morbidity and mortality. Despite these benefits, SGLT-2is are associated with certain adverse effects (AEs), particularly genitourinary (GU) events, which can lead to treatment discontinuation in some patients. Preventing these AEs is essential for maintaining the cardiorenal benefits of SGLT-2is.

Introduction: Melioidosis is caused by Burkholderia pseudomallei and primarily affects non-vascular organ systems. We present a case of a melioidotic penetrating aortic ulcer (PAU) with unusual clinical features of vascular infection. The patient was successfully treated with a single-stage neoaortoiliac system procedure, highlighting the challenges in managing melioidotic aortic infections.
