Acute renal infarction occurs usually secondary to thromboembolism rather than thrombosis, with atrial fibrillation being the main predisposing factor. Its non-specific presentation, being similar to renal colic and pyelonephritis, often leads to diagnostic and treatment delays. Prompt diagnosis and treatment are crucial for the outcome, as the precise therapeutic window remains unclear. Renal Doppler ultrasound is the optimal initial diagnostic study, which, if inconclusive, should be followed by contrast-enhanced CT. Despite the lack of specific guidelines, treatment is mainly based on anticoagulation and percutaneous endovascular revascularization therapy; the latter includes pharmacomechanical thrombectomy; intra-arterial thrombolysis alone or in combination with thrombus aspiration; and angioplasty, with or without stenting. We present a case of renal thromboembolism diagnosed early during the postoperative period in a patient with a single functioning kidney. Renal arterial flow restoration was achieved by means of transcatheter thrombus aspiration. This is the first report showing the effectiveness of this procedure alone as an alternative to those used so far.

Introduction/aim: The partial deployment technique (PDT) is an unconventional option of T-branch deployment to allow target arteries (TAs) cannulation/stenting from the upper arm access, in case of narrow (NPA: <25mm) or severely angulated (APA: >60°) aorta. Aim of this study was to report outcomes of the endovascular repair of complex aortic (c-AAAs) and thoracoabdominal (TAAAs) aneurysms by T-branch and PDT.

Methods: All consecutive patients underwent urgent endovascular repair of c-AAAs and TAAAs by T-branch (Cook-Medical, Bloomington, IN, US) and PDT from 2021 to 2023 were analyzed.

: ST-segment elevation myocardial infarction (STEMI) remains a leading cause of mortality worldwide, primarily caused by acute thrombosis over atherosclerotic plaques. Simultaneous acute thrombosis in two coronary arteries is an exceptionally rare event. This report highlights a unique case of STEMI associated with cardiogenic shock due to dual coronary artery thrombosis and provides insights from a literature review on this rare condition.

Essential thrombocythemia (ET) is a type of myeloproliferative neoplasm (MPN) disorder characterized by persistent thrombocytosis and characterized by frequent association with cellular genetic alterations. The 10%-15% of ET that is not associated with genetic abnormalities is known as triple-negative essential thrombocythemia (TNET). A common complication observed in around 20% of ET patients is the development of acquired von Willebrand disease (AvWD).

Background: Percutaneous mechanical aspiration thrombectomy is increasingly being employed as a less invasive alternative for managing right heart masses, including clot in transit.

Aims: We aimed to analyze trends in the use of catheter-directed aspiration (CDA) for right heart masses. Additionally, we analyzed in-hospital outcomes of percutaneous versus surgical approaches for patients with CIT and PE.

Article Synopsis
  • Coronary artery ectasia (CAE) is a rare condition usually found during coronary angiography and can increase the risk of blood clots.
  • A 40-year-old male with acute coronary syndrome experienced a complete blockage in his ectatic left circumflex artery and the dominant right coronary artery.
  • After thrombus aspiration restored normal blood flow, the case highlights the difficulties in treating acute coronary syndrome in patients with CAE and stresses the need for further research on effective treatment methods.
