Retrograde Use of Gore Hybrid Vascular Graft for a Complex Carotid Tandem Lesion.

Ann Vasc Surg

Ospedale Civile Cardinal Massaia, Division of Vascular and Endovascular Surgery, Asti, Italy.

Published: February 2019

Background: Successful treatment of tandem lesions of carotid arteries may represent a challenging goal for the vascular specialist. In the "endovascular era," a hybrid approach may represent a viable option.

Case Report: We describe the case of a 65-year-old patient with severe postcarotid endarterectomy recurrent stenosis of the internal carotid artery (ICA) associated with primitive stenosis of the proximal common carotid artery (CCA) treated with retrograde stenting and carotid bypass using the Gore Hybrid Vascular Graft (GHVG).

Conclusions: We demonstrated the effectiveness of the hybrid technique using GHVG in treating carotid tandem lesions involving CCA and ICA.

Article Synopsis
  • * The study looked at 40 CTD patients who had TEVAR for thoracoabdominal aortic aneurysms or aortic dissections between February 2014 and April 2021, revealing most had previous aortic interventions and a significant number faced complications post-surgery.
  • * Results showed a high reintervention rate (62.5%), with reinterventions typically occurring within the first year, and indicated ongoing risks for aortic-related issues, highlighting challenges in treating CTD patients

Objective: Postoperative gastrointestinal hemorrhage (GIH) following mesenteric revascularization when performed either openly (OR) or endovascularly (ER) has been clinically observed but not reported. The aim of the study is to assess the incidence and predictors of GIH in patients undergoing mesenteric revascularization.

Methods: This was a single-center retrospective review of consecutive patients treated with open or endovascular mesenteric revascularization from 2009 to 2019.

Background: Health technology assessment (HTA) organizations generate guidelines to inform healthcare practices toward improved health outcomes. This review sought to identify and classify outcomes of guidelines from HTA organizations within published research.

Methodology: We performed a systematic mixed studies review of empirical studies that (a) referred to a published guideline from an HTA organization and (b) reported an outcome resulting from a guideline.

Article Synopsis
  • Hybrid stage I palliation is a procedure used mainly for high-risk patients with single ventricle heart disease, serving various roles such as a starter treatment or a bridge to more complex surgeries or transplants.
  • A survey sent to congenital heart centers in the U.S. and Canada revealed that about 45 out of 106 centers perform this type of palliation, with a majority using pulmonary artery bands and ductal stents for management.
  • There is significant variation in the practices and indications for hybrid stage I palliation among centers, highlighting the need for multicenter studies to establish standardized protocols and improve patient outcomes.

Objective: This study aimed to compare the completion of gate cannulation task performed by participants of varying experience using fluoroscopy, the Intraoperative Positioning System (IOPS)-a United States Food and Drug Administration-cleared endovascular navigation system that has been developed to reduce dependence on fluoroscopy-or an investigational augmented reality electromagnetic navigation technology based on IOPS.

Methods: The task consisted in the cannulation of the gate of a GORE Excluder AAA endoprosthesis bifurcated aortic stent graft (W.L.
