A leadless pacemaker (LP) is a modern alternative to a transvenous pacemaker, allowing certain complications to be avoided; however, some cannot be eliminated. To highlight the essential role of advanced speckle-tracking echocardiography (STE) in diagnosing pacing-induced cardiomyopathy (PICM) caused by an LP. A 79-year-old male, after LP implantation a year earlier, was admitted due to heart failure (HF).

Introduction: There is no prior report of an Aveir leadless pacemaker implantation into the atrial appendage via the internal jugular vein.
Case: A 44-year-old female patient with history of multiple ablations for sinus node dysfunction presented with symptomatic bradycardia. The patient had femoral veins <9mm, chronic pain at the femoral vein insertion sites, as well as a recent car accident with persistent leg pain due to femoral fractures.
Conduction disturbances following transcatheter aortic valve replacement (TAVR) sometimes require permanent pacemaker implantation. However, little is known about the pacing-induced cardiomyopathy (PICM) in leadless pacemaker (LP) after TAVR. Here we present a case of heart failure due to PICM 6 months after TAVR which was successfully treated with LOT-CRT upgrade.
