Systematic Comparison of the Influence of Cool Wall versus Cool Roof Adoption on Urban Climate in the Los Angeles Basin.

This study for the first time assesses the influence of employing solar reflective "cool" walls on the urban energy budget and summertime climate of the Los Angeles basin. We systematically compare the effects of cool walls to cool roofs, a heat mitigation strategy that has been widely studied and employed, using a consistent modeling framework (the Weather Research and Forecasting model). Adoption of cool walls leads to increases in urban grid cell albedo that peak in the early morning and late afternoon, when the ratio of solar radiation onto vertical walls versus horizontal surfaces is at a maximum. In Los Angeles County, daily average increase in grid cell reflected solar radiation from increasing wall albedo by 0.80 is 9.1 W m, 43% of that for increasing roof albedo. Cool walls reduce canyon air temperatures in Los Angeles by 0.43 K (daily average), with the peak reduction (0.64 K) occurring at 09:00 LST and a secondary peak (0.53 K) at 18:00 LST. Per 0.10 wall (roof) albedo increase, cool walls (roofs) can reduce summertime daily average canyon air temperature by 0.05 K (0.06 K). Results reported here can be used to inform policies on urban heat island mitigation or climate change adaptation.

Background: With recent advances in single cell technology, high-throughput methods provide unique insight into disease mechanisms and more importantly, cell type origin. Here, we used multi-omics data to understand how genetic variants from genome-wide association studies influence development of disease. We show in principle how to use genetic algorithms with normal, matching pairs of single-nucleus RNA- and ATAC-seq, genome annotations, and protein-protein interaction data to describe the genes and cell types collectively and their contribution to increased risk.

Objective: To reflect on factors that may have led to the widespread implementation of gender affirming care (GAC) for minors by psychiatric clinical leaders despite the absence of a robust evidence base and the known risks of harm.

Conclusions: The progressive rejection of psychodynamic thinking by the profession of psychiatry may have contributed to psychiatrists failing to question key aspects of GAC for minors. Further, numerous unconscious factors potentially contribute to the foreclosure of thinking about the risks of gender medicine.

Strong evidence supports the importance of rapid sequence or simultaneous initiation of quadruple guideline-directed medical therapy (GDMT) for heart failure with reduced ejection fraction (HFrEF) for substantially reducing risk of mortality and hospitalization. Barring absolute contraindications for each individual medication, employing the strategy of rapid sequence, simultaneous, and/or in-hospital initiation at the time of HF diagnosis best ensures patients with HFrEF have the opportunity to benefit from proven medications and achieve large absolute risk reductions for adverse clinical outcomes. However, despite guideline recommendations supporting this approach, implementation in clinical practice remains persistently low, with less than one-fifth of eligible patients being prescribed the quadruple GDMT regimen.
