Guide to Enhancing Swallowing Initiation: Insights from Findings in Healthy Subjects and Dysphagic Patients.

Purpose Of Review: Difficulty in initiating swallowing is one of the main symptoms of oropharyngeal dysphagia. Therefore, enhancing swallowing initiation is an important approach for the treatment of oropharyngeal dysphagia. This review aims to introduce recent approaches to enhancing swallowing and to discuss their therapeutic potential.

Recent Findings: Both central interventions such as non-invasive brain stimulation and peripheral interventions such as electrical stimulation to peripheral tissues are conducted to enhance swallowing. Recent studies have paid more attention to generating neuroplasticity to produce long-lasting facilitative effect on swallowing.

Summary: Transcranial magnetic stimulation (TMS), transcranial direct current stimulation (tDCS), pharyngeal electrical stimulation (PES), transcutaneous electrical stimulation, and somatic and chemical stimulation were introduced. Considerable evidence supports the therapeutic potential of TMS and PES. Other approaches need further studies to verify their efficacy (e.g., duration of the effect and a limit of effectiveness) and/or possible risk of adverse effects.

Patients: Three residents of a long-term care home volunteered to participate in a rehabilitation program involving a virtual reality blowgun game. The participants played the game 5 days a week as a new exercise routine. After 4 weeks of intervention, tongue pressure and forced expiratory volume in 1 s improved in all participants.

Decreased saliva production due to salivary gland damage can result in difficulty speaking and swallowing, significantly reducing quality of life for head and neck cancer patients receiving radiotherapy. It is therefore imperative that treatment options are available to mitigate the effects of these debilitating side effects. D-limonene, a naturally occurring terpene, has shown protective effects on saliva production during radiotherapy treatment of mice, however the lipophilic nature of the molecule has necessitated a high oral dose to facilitate sufficient absorption.

Dysphagia, a swallowing disorder, requires continuous monitoring of throat-related events to obtain comprehensive insights into the patient's pharyngeal and laryngeal functions. However, conventional assessments were performed by medical professionals in clinical settings, limiting persistent monitoring. We demonstrate feasibility of a ubiquitous monitoring system for autonomously detecting throat-related events utilizing a soft skin-attachable throat vibration sensor (STVS).

: Oropharyngeal dysphagia is a common swallowing disorder, characterized by difficulties in moving food and liquids from the mouth to the esophagus; it is particularly prevalent among older adults with neurological conditions. This study aimed to evaluate the effectiveness of a short-term complex treatment protocol combining dietary modifications, swallowing exercises, and transcutaneous neuromuscular electrostimulation in reducing the oropharyngeal dysphagia severity and aspiration risk among geriatric patients. : A total of 64 participants aged 60 and older, with oropharyngeal dysphagia, at LSMU Kaunas Hospital between May 2021 and April 2023, were included in the study after excluding those with significant comorbidities.
