Genital lymphatic filariasis (LF) is a condition that can present both acutely and chronically, complicating its diagnosis due to nonspecific symptoms. This case report describes an 11-year-old boy who presented with symptoms of acute scrotum. Initial Doppler ultrasound suggested acute epididymo-orchitis; however, despite conservative management, the symptoms persisted, prompting surgical exploration.

The surgical repair of giant inguinal hernias with loss of domain, defined as the relocation of the majority of the intestine into the hernia sac, poses a significant challenge. In the majority of cases, a combination of different surgical techniques with the placement of multiple meshes is necessary to achieve reduction of such complex hernias. The reduction of chronic giant hernias can increase the risk of abdominal compartment syndrome or cardiopulmonary complications.

Hydrocele of the Canal of Nuck is a rare inguinal-scrotal swelling that occurs primarily as a chronic painful lump in childhood or in young women. Here, we present a rare case of the same, presenting at an extreme of age and with a short history. An 85-year-old female patient presented with a progressive inguinal swelling for 2 months in a known case of decompensated cirrhosis of liver in form of ascites and portal cavernoma.
