AI Article Synopsis

  • There is ongoing debate about the best time to start dialysis for patients with chronic kidney disease (CKD), and this study explores how initiation timing affects long-term outcomes in end-stage renal disease (ESRD) patients.
  • A total of 294 patients were analyzed and classified into four groups based on their estimated glomerular filtration rate (eGFR) when they began hemodialysis, with a median eGFR of 5.43 ml/min.
  • Results showed that factors like comorbidities were more significant predictors of all-cause mortality than eGFR at the time of dialysis initiation, indicating that having a higher eGFR before starting hemodialysis did not improve long-term prognosis.

Article Abstract

There are a lot of controversies pertaining to correctness of timing for the initiation of dialysis in chronic kidney disease (CKD) patients. The present study was conducted to examine the potential association of initiation timing of dialyses with long-term prognosis in CKD patients. In total, 294 patients confirmed as end-stage renal disease (ESRD) were included as study subjects. According to the estimated glomerular filtration rate (eGFR) at initiation time, the patients were classified into four groups based on eGFR: ≥10.5, 8-10.4, 6-8 and <6 ml/min × (1.73 m). The primary outcomes were defined by all-cause mortality. The median eGFR of the 294 patients at initiation time was 5.43 (2.27-13.92) ml/min × (1.73 m). The patients with lower eGFR at the start of hemodialysis had a higher proportion of Charlson comorbidity index (CCI) scores of 0-2 and had lower hematocrit. The multivariate Cox regression analysis indicated that CCI, cerebrovascular diseases and chronic obstructive pulmonary disease were significantly associated with all-cause mortality, but not eGFR at the dialysis initiation. Furthermore, stratified analyses confirmed elevated eGFR that had no advantage on long-term prognosis. The present findings have shown that the prevalence of anemia, hyperuricemia and calcium and phosphorus metabolism disorders of patients with low eGFRs at the initiation of hemodialysis was higher in comparison to the patients with high eGFRs. Therefore, the long-term prognosis of patients with high eGFRs prior to hemodialysis was not improved.

Avascular necrosis (AVN) of the hip, a rare cause of pelvic pain in the third trimester of pregnancy, often presents with nonspecific symptoms that resemble common musculoskeletal conditions. This ambiguity, coupled with concerns about the safety of magnetic resonance imaging (MRI) during pregnancy, can hinder timely diagnosis. We report a unique case of a 32-year-old primigravida diagnosed with a hip fracture and bilateral AVN of the femoral head in the immediate postpartum period, a complication seemingly not previously documented in pregnant patients.

Bullous type 2 lepra reactions are a rare initial presentation in leprosy that can complicate the disease with vesiculo-bullous lesions. Early recognition and differentiation from other bullous disorders are critical for timely corticosteroid and multidrug therapy initiation to improve patient outcomes.

Metyrapone is commonly used in the initial management of Cushing's syndrome to reduce hypercortisolemia, but its optimal dosage and timing can vary significantly between patients. Currently, there are limited guidelines on adjustment methods for its administration to individual needs. This study aimed to evaluate responsiveness of each patient to metyrapone and identify the patient characteristics associated with the indices of cortisol responsiveness following a low-dose metyrapone.

Background: Chondrosarcoma is the second most common bone malignancy. Chondrosarcoma is considered a surgical disease, and the role of surgical margin is important since local recurrence is known to decrease survival. The aim of this study was to investigate the timing of local recurrence and the size of local recurrence in post local recurrence survival in chondrosarcoma.
