Background: Nodal only recurrence post radical prostatectomy (RP) is increasingly recognised in the PSMA scan era. Management is controversial with a curative approach usually incorporating prostate bed and nodal irradiation (PB + NRT) in combination with long-term hormonal therapy. It is unknown whether omitting prostate-bed irradiation (PBRT) is safe in a subgroup of these patients.

In managing this complex case, a Penrose drain was utilized as a spacer during artificial urinary sphincter cuff removal to preserve the urethral space for future sphincter re-implantation. This report highlights the novel application of a Penrose drain as a spacer in urological surgery and its benefits in minimizing tissue contraction and preserving urethral health.

Background: Prostate bed (PB) motion may lead to geographical miss of the target volume in post-prostatectomy radiotherapy (RT). Optimal clinical target volume (CTV) to planning target volume (PTV) margins prevent geographical miss and unnecessary irradiation of normal tissue. There is little data available informing appropriate CTV to PTV margins in the post-prostatectomy setting.
